Cubitus Varus / Gunstock Deformity Elbow ; Lateral Closing Wedge Corrective Osteotomy ( Surgical technique and Steps)

In this video, we demonstrate a surgical procedure performed on a 10-year-old patient presenting with a malunited supracondylar fracture resulting in cubital varus deformity. This condition, also known as "gunstock deformity," can significantly impact arm function and appearance if not corrected.

Procedure Highlights:
- The patient underwent a lateral closing osteotomy to correct the alignment and restore a normal carrying angle.

- The surgery involved carefully removing a wedge of bone laterally, allowing for precise realignment of the arm.

- Fixation was achieved using k-wire to stabilize the bone during healing.

In the Before and After images, you can clearly see the difference in alignment, with successful restoration of a natural arm position. This correction will not only improve the child's functionality but also enhance arm aesthetics.

Topics Covered:
- Preoperative planning and imaging
- Step-by-step guide to lateral closing osteotomy
- Fixation techniques and post-op considerations

Rotator cuff injuries are common, especially among athletes and individuals who engage in repetitive overhead activities. The rotator cuff is a group of muscles and tendons that stabilize the shoulder joint and allow for a wide range of arm movements. Injuries can range from mild inflammation or tendinitis to partial or complete tears.

𝐓𝐲𝐩𝐞𝐬 𝐨𝐟 𝐑𝐨𝐭𝐚𝐭𝐨𝐫 𝐂𝐮𝐟𝐟 𝐈𝐧𝐣𝐮𝐫𝐢𝐞𝐬
:
𝐓𝐞𝐧𝐝𝐢𝐧𝐢𝐭𝐢𝐬:
Inflammation of the tendons is often caused by overuse.

𝐁𝐮𝐫𝐬𝐢𝐭𝐢𝐬:
Inflammation of the bursa, a fluid-filled sac that helps reduce friction between the tendons and bones.

𝐓𝐞𝐚𝐫𝐬:
Partial or complete tearing of the rotator cuff tendons, often due to injury or degeneration over time.

𝓢𝔂𝓶𝓹𝓽𝓸𝓶𝓼:
Pain, especially when lifting the arm or performing overhead activities.
Weakness in the shoulder.
Limited range of motion.
Clicking or popping sensations in the shoulder.

𝐓𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭 𝐎𝐩𝐭𝐢𝐨𝐧𝐬:

1. Non-Surgical Treatments:

Rest and Activity Modification:
Avoid activities that exacerbate pain.

Physical Therapy:
Strengthening and stretching exercises help improve mobility and stabilize the shoulder joint.
Non-steroidal anti-inflammatory Drugs (NSAIDs):
These can help reduce severe inflammation and pain but are usually used as a short-term solution.

2. Surgical Treatments (recommended for severe tears or cases unresponsive to conservative management):

Arthroscopic Repair:
A minimally invasive procedure where small incisions are made, and the torn tendons are reattached to the bone.

Open Surgical Repair:
A traditional approach involves a larger incision to repair the tear.

Tendon Transfer:
Used when the rotator cuff is too damaged to repair; a nearby tendon is moved to take over the function of the rotator cuff.

3. Rehabilitation:
Post-injury or post-surgery rehabilitation plays a crucial role in recovery:

Early Phase:
Focuses on pain management and gentle movements to prevent stiffness.
Strengthening Phase: Gradually increases the load on the shoulder to rebuild strength and mobility.

Return to Activity:
The final stage of rehabilitation, is where patients are guided to return to normal activities or sports, with a focus on preventing re-injury.
Effective treatment depends on the severity of the injury and patient-specific factors, so a personalized approach is crucial for optimal recovery.
