Pe**le fracture is urology emergency in which patient during in*******se experiences popping sound followed by immediate detumescence and swelling with or without bend in pe**le shaft. In rare cases it may be associated with urethral injury as well shown by urinary retention and urethral bleeding/hematuria. In Time surgical repair prevents future erectile dysfunction and urethral stricture/chordee. A case of pe**le fracture and urethral injury repaired surgically.

A 16 year male adolescent boy with acute onset left severe scrotal pain treated as Epididymitis/Orchitis by GP for 2to3 days but pain persisted rather scrotal swelling developed. Referred to me on 3rd day. His scrotal doppler ultrasound revealed absent blood supply to te**is. Surgical exploration done immediately but non-viable te**is found unfortunately resulted in Orchidectomy. This organ loss can be prevented by followings: 1. history shows no sexual contact, 2. Pain usually not relieved by analgesics 3. Careful physical examination shows high riding Te**es 4. May have previous such pain attack 5. Age usually around 10 to 20 years. If suspected torsion get immediate scrotal doppler ultrasound to look for compromised or absent blood supply. Immediate surgical exploration then can save te**is with contralateral normal testicular fixation also.
