Often, in response to concerned parents, the treating physician prescribes medications. A reminder that, specially in children, one should give careful thought to whether this is necessary. Will it benefit the child in any way??Are there side-effects?? This applies to ALL medicines, not only antibiotics. Doctors need to be more aware and careful. Equally the parents should also recognize that in some illnesses, medication may actually be harmful and therefore not insist on getting medicines.

There is a new disease spreading; the mpox or monkeypox. The affected persons have fever body aches, headache and a rash of blisters with fluid in them. The disease is very contagious and spreads from person to person, mainly through touch. If not sure of any such rash in any member of your family or amongst your friends, please check with your doctor. Sometimes pimples and boils seen commonly during the rainy season can confuse parents. These are bacterial and can be treated easily.

Quite often, the family members of a child ask you for “a good medicine for cough” to be used whenever the child needs it. You must explain that cough is just a symptom, a protective mechanism of the body. The underlying condition maybe a simple cold or an allergy or asthma or some condition as serious as a pneumonia. The treatment therefore will be different for each condition. This would be the most appropriate way to prescribe.

It is generally warmer than usual and we must protect our children from excessive heat and the sun. Best is to let them get outdoors early morning or late evenings or allow them to play in shade. Loose cotton clothing is best, with the head being protected if possible. Ensure plenty of liquids, specially lemonade, lassi, yogurt. Fruits and vegetables should be encouraged.

For gastrointestinal problems, worsening means increased vomiting, larger, more watery and more frequent stools, along with decrease in feeding, worsening fever, or becoming lethargic. If the child starts breathing fast or is not passing urine, do consult your doctor or take the child to the nearest emergency room.

For respiratory symptoms, worsening means increasing severity of cough, rapid breathing, breathing difficulty, increasing fever, becoming lethargic or not being able to eat or feed properly. When you visit your pediatrician, the doctor often tells you to either call or visit if the child gets worse. It is important for you to ask what the doctor means by getting worse. It is also the responsibility of a doctor to explain how parents get to know that their child is worse and may need urgent attention.
I shall give you some examples in the coming posts.
