Dr Kaleem Ullah is currently working as Chairman of department of Thoracic surgery at Nishtar hospital multan. Building upon the legacy of his mentor. He is striving to provide quality health services regarding thoracic surgery to not only this region but his mastery in upper GI reconstruction has earned him name till neighbouring countries. Probably the most Colonic interpositions under his belt worldwide, he is one of the pioneers of VATS Esophagus mobilisation in South Punjab and has authored a number of research papers in impactful journals.
Under his mentorship upcoming fellows are springing up across punjab to fill the much needed void of Thoracic surgery in this region.

Prof. Dr Iftikhar Khan is one of the pioneers of thoracic surgery not just in Multan but the country. He is FRCS(Uk) and FCPS (Thoracic surgery). Under his mentorship a number of General and Thoracic Surgeons have been trained and are now accomplished in their respective domains worldwide. He has served as principal at Nishtar medical College. His contributions to Thoracic Surgery include innovative procedures like PVC Stenting of Esophagus and Bifid Stomach tube. He is an author of Housejob Guide -'SURVIVING CLINICAL INFANCY', a well received and rare book on Clinical guidance for young and upcoming doctors. He spearheaded the creation of Punjab's first public sector Thoracic center at Nishtar recognized for training by CPSP at Multan. Currently practicing at MASH Multan and Chenab hospital, he is an active educational content creator on YouTube.

Department of Thoracic Surgery regularly conducts classes for improving clinical skills and knowledge of its Houseofficers and Post graduate residents. These are two class sessions from last week where trainees and houseofficers were familiarized with sutures and radiology.

Patient feedback. !
A patient presented to us with history of accidental acid intake causing esophageal stricturing and severe inflammation along with destroyed stomach. Patient developed absolute dysphagia i.e was not even able to swallow his own saliva. Temporary Gastrodudenal dysjunction was performed i.e stomach was separated from dudenum and both attached to external drains. Dudenal drain was used for feeding purpose for 3.5 yrs till the inflammation settled and then colonic interposition was performed i.e colon graft was taken and anastamosis made with esophagus and small gut bypassing the natural esophagus. So now patient is able to take orally after many years.

Members of Department of Thoracic Surgery , Nishtar Hospital recently attended a 3 day First International Minimally invasive chest wall surgery conference by PROF. DR. Wenlin Wang. Dr
Wenlin is the founder of first ever chest wall surgery department in the world and has many innovative procedures to his credit. He was here to familiarize Thoracic surgeons of Pakistan with his latest advancements in chest wall surgery with hands-on experience.

3 cases of Foreign body inhalation presented to us simultaneously.
Case 1 - A 7 year boy from LAYYAH had inhaled plastic cover of back side of pencil which was radiolucent and was not appreciated on radiology preoperatively however the child was symptomatic with bouts of cough with hemoptysis and shortness of breath. Bronchoscopy revealed the foreign body to be in Left main Bronchus which was retreived.

Case 2- A 12 year old girl from ALIPUR had inhaled a hairpin which was seen in main trachea on x-ray and was retrieved via bronchoscopy.

Case 3- A 1 year old boy from JHANG had inhaled a stone-chip and was lodged in right main bronchus according to C-x-ay which was retrieved from right main bronchus.

Since its inception, the department has evolved into a center of excellence offering both elective and emergency thoracic surgical services. It manages a wide spectrum of thoracic diseases including pulmonary resections, pleural diseases especially empyema thoracis, mediastinal masses, upper GI surgeries, colonic interposition, tracheal resection and reconstruction, diaphragmatic surgeries, chest wall reconstruction, chest wall deformity and chest trauma.

The department has also embraced minimally invasive techniques, particularly Video-Assisted Thoracoscopic Surgery (VATS), significantly improving surgical precision, reducing postoperative complications, and enhancing recovery outcomes for patients

A 15 year boy presented with history of cough for last 2 years. Upon investigation it was revealed that he had inhaled a battery cell 2 years ago. On radiology it was lodged in Left main bronchus . On Bronchoscopy there was complete stenosis (closure) of Left main bronchus and battery cell could not be seen. So Left thoracotomy was planned. Bronchus opened and Battery cell removed and Bronchial resection and anastamosis was performed (i.e closed part of left lung's main airway was removed and remaining part restitched)

The Department of Thoracic Surgery, Nishtar Hospital Multan, was established in 2002 by Prof. Dr. Iftikhar Hussain Khan with the primary objective of providing specialized surgical care to patients suffering from diseases of the esophagus, stomach, trachea, lungs, pleura, mediastinum, diaphragm and chest wall. Its establishment marked a significant milestone in the development of advanced thoracic surgical services not only in South Punjab but also serve the community of lower Sindh, KPK and Balochistan addressing a critical gap in specialized patient care within the region.
