Intravenous to Oral (IV to PO) Switch Guidelines

IV to oral switch is the prompt conversion of IV antibiotic therapy tooral. Patients may be considered candidates for switching from IV toOral therapy once the patient has shown clinical improvement and ismedically stable.

Considerations for IV to Oral switch - ABCD

🎯 A- A febrile >24hrs (haemodynamically stable with no sign of fever) Patients temperature between 36-38C for 24 hrs.

🎯B- Able to take oral medications; have a functional GI tract with no mal-absorption and no interaction with other medications

🎯C- Clinically improving (markers normalizing)
👉 Improving signs and symptoms of infection and patient’s
general condition getting better.
👉Clinical markers improving.
👉No unexplained tachycardia (heart rate

✅ Alhamdulilah for the first time in kpk 1st batch of Pharmacotherapy attachment program for three months has been completed under supervision Department of Clinical Pharmacy MTI-HMC Hospital Peshawar.

📌 Proper structured training were given to Clinical interns in different wards under supervision of concern ward Clinical Pharmacist.

يعني د چراثيم کش دوائي خلاف مقاومت يا مزاحمت زموږ د ناپوهۍ او بي غوري له امله مخ په زياتيدو دي او په راتلونکي وخت کښې زموږ ټولو د پاره ډير خطرناک ثابتې دي شي.
د دي له امله WHO هر کال د 18 نومبر سخه تر 24 نومبر د Antibiotics Resistance Awareness week يعني د چراثيم کش دوائي خلاف مقاومت يا مزاحمت پوهي اونۍ په حيث لمانځي.
