How High Blood Pressure Causes Chronic Kidney Disease (CKD)

High blood pressure, also called hypertension, is one of the leading causes of Chronic Kidney Disease worldwide. The kidneys are delicate filtering organs, and when blood pressure remains high for a long time, it puts constant strain on them.

How It Happens
1. Damages blood vessels
The kidneys filter blood through tiny blood vessels. High pressure damages and narrows these vessels, reducing blood flow.
2. Weakens filtering ability
With reduced blood flow, the kidneys cannot effectively remove waste, salt, and extra fluid from the body.
3. Creates a vicious cycle
As the kidneys weaken, fluid and waste build up in the body, which can further raise blood pressure — worsening the damage.

Signs to Watch For
• Swelling in feet, ankles, or hands
• Foamy urine (protein loss)
• Fatigue and weakness
• Headaches or dizziness

Bottom line: High blood pressure silently injures the kidneys. Early control and care are the best protection against CKD.

Symptoms of kidney disease

Nausea and vomiting are recognized symptoms of kidney disease, particularly in later stages or during acute kidney failure. Here’s why they occur:

1. Build-up of waste products: When the kidneys are not functioning properly, they can’t filter waste from the blood effectively. This leads to a build-up of toxins (uremia), which can irritate the gastrointestinal tract and trigger nausea and vomiting.
2. Electrolyte imbalances: Kidney dysfunction can cause imbalances in electrolytes like potassium, sodium, and calcium, which may affect stomach and brain function, leading to nausea.
3. Medications: Some medications prescribed for kidney disease or related conditions (like high blood pressure) can also cause nausea as a side effect.

نیفرالوجی (گردے کی بیماریوں کے مطالعہ) میں سٹیریئڈز (corticosteroids) کا کردار کئی بیماریوں کے علاج میں انتہائی اہم ہے، خاص طور پر وہ بیماریاں جن میں مدافعتی نظام (immune system) کا عمل دخل ہوتا ہے۔ سٹیریئڈز کا بنیادی مقصد سوزش (inflammation) کو کم کرنا اور گردے کو مزید نقصان سے بچانا ہوتا ہے۔

:نیفروٹک سنڈروم (Nephrotic Syndrome):بچوں میں نیفروٹک سنڈروم اکثر "Minimal Change Disease" کی وجہ سے ہوتا ہے، اور ان کا سٹیریئڈز کے ساتھ علاج عام طور پر مؤثر ہوتا ہے۔ سٹیریئڈز سے حساس نیفروٹک سنڈروم (Steroid-sensitive nephrotic syndrome) کے مریض اکثر مکمل ریکوری کرتے ہیں، لیکن کچھ مریض سٹیریئڈز پر منحصر (dependent) یا مزاحم (resistant) ہو جاتے ہیں، جنہیں مزید امیونوسپریسیو دوا کی ضرورت پڑ سکتی ہے
۔گلومیرولونفرائٹس (Glomerulonephritis):جیسے کہ IgA نیفروپیتھی، میمبرینس نیفروپیتھی یا lupus nephritis میں، سٹیریئڈز کا استعمال گردے کی سوزش کو کم کرنے کے لیے کیا جاتا ہے۔ البتہ، حالیہ مطالعات جیسے STOP-IgAN اور TESTING نے یہ دکھایا کہ اگرچہ سٹیریئڈز پیشاب میں پروٹین کی مقدار کو کم کر سکتے ہیں، لیکن یہ ہمیشہ گردے کے کام (eGFR) کو بہتر نہیں بناتے اور ان کے مضر اثرات جیسے انفیکشن، وزن میں اضافہ اور ذیابطیس کی شدت بڑھ سکتی ہے
۔گردے کی سوزش (Acute Interstitial Nephritis):اگر یہ سوزش ادویات (مثلاً NSAIDs) کی وجہ سے ہو، تو بنیادی دوا کو روکنا ضروری ہوتا ہے۔ کچھ کیسز میں سٹیریئڈز استعمال کیے جا سکتے ہیں، خاص طور پر جب سوزش شدید ہو یا گردے کی کارکردگی تیزی سے خراب ہو رہی ہو، لیکن ہر مریض میں فائدہ یقینی نہیں
۔نایاب بیماریاں (Rare Diseases):جیسے Chinese Herb Nephropathy یا Aristolochic Acid Nephropathy میں، سٹیریئڈز کا استعمال کچھ مطالعات میں فائدہ مند پایا گیا ہے، لیکن یہ علاج کے لیے بنیادی حل نہیں بلکہ ضمنی مدد کے طور پر استعمال ہوتے ہیں
۔سٹیریئڈز کے ممکنہ نقصان:کچھ حالات میں سٹیریئڈز کا استعمال نقصان دہ ہو سکتا ہے، جیسے کہ سکلرودرما رینل کرائسز (Scleroderma renal crisis) میں، جہاں سٹیریئڈز دینے سے مریض کی حالت مزید بگڑ سکتی ہے۔ اس لیے صحیح تشخیص کے بغیر سٹیریئڈز دینا خطرناک ہو سکتا ہے
نوٹ
:سٹیریئڈز نیفرالوجی میں ایک مؤثر مگر پیچیدہ دوا ہیں، جن کا استعمال صرف اس وقت ہونا چاہیے جب مرض کی درست تشخیص ہو اور ماہر نیفرالوجسٹ کی نگرانی میں ہو۔ ان کا استعمال ہمیشہ فائدہ نہیں دیتا اور بعض اوقات نقصان دہ بھی ثابت ہو سکتا ہے۔
