Femoral Neck Fractures:

Why TYPE Matters?
The femoral neck is the weakest part of the femur - nature’s trade-off for bipedal walking. 60% of all are intracapsular, 80% are displaced and they have a notorious reputation: 20-30% 1-yr mortality rate in elderly patients. Types:
🔹Capital fractures: occur at femoral head, are rarest but devastating with a 90% osteonecrosis risk; often require immediate surgical intervention necessitating femoral head replacement vs. repair
🔹Subcapital: at head-neck junction and follow the famous Garden Classification system: Type I - valgus impacted/relatively stable, while Type IV represent complete displacement w/ poor prognosis; 50% osteonecrosis risk due to disrupted blood supply from the medial femoral circumflex artery
🔹Transcervical: pass through the femoral neck’s middle portion. The Pauwels classification becomes critical - fractures angled >50 degrees from horizontal show increased instability + higher failure rates. Emergency reduction within 6hrs may preserve femoral head viability in younger patients
🔹Intertrochanteric: between the trochanters and represent the “good news” story. Being extracapsular, they maintain excellent blood supply with only ~10% osteonecrosis risk. These respond well to sliding hip screws or intramedullary nails - much better healing potential
🔹Subtrochanteric: within 5cm below lesser trochanter, present unique challenges. High mechanical stress + powerful muscle forces create deforming forces that resist reduction. Atypical fractures from long-term bisphosphonate use preferentially occur in this location, creating characteristic horizontal “chalk stick” breaks
Treatment approaches vary dramatically by location/patient age. Young patients require emergency ORIF to preserve native femoral head. Elderly patients w/ displaced fractures often benefit from prosthetic replacement (hemiarthroplasty or total hip replacement). Recognition of fracture patterns guides surgical planning + sets realistic expectations for patients.
