A doctor cleared $650,000 in medical bills for 200 cancer patients.

He chose compassion over payment.

In a remarkable act of generosity, Dr. Omar Atiq, an oncologist from Arkansas, decided to erase the outstanding bills of his cancer patients at the end of his career. Many were struggling financially while battling life-threatening illness. Instead of sending final payment reminders, he sent letters telling them their debts had been forgiven entirely.

Dr. Mashhood Hamza graduated in Medicine in 2016 from Bahria University, Islamabad. He continued to pursue his passion in General Surgery, with a strong focus on precision and accurate diagnosis. Dr. Hamza has authored a variety of research articles on surgery and infections, as well as a book on Dengue Fever.

He earned his MACP in Medicine from the American College of Physicians and has had the privilege of working under Fellows of the Royal College of London, Glasgow, Dublin, and Edinburgh. Dr. Hamza practices according to US medical codes and standards. Currently, he is preparing for the MRCS Part II examination, further advancing his expertise in surgery.

Burn Injury

Some take home messages

1. The immediate treatment, is to cool the burn area, like running pipe water over the area for 20 minutes

2. Calculate the burn area ONLY the partial thickness (2nd degree) and full thickness (3rd degree) areas. The superficial thickness (1st degree) are not included in the calculation.

3. Categories of burn injuries
a. 1st degree (superficial thickness - epidermis) : just like sunburn, some redness
b. 2nd degree (superficial partial thickness – superficial dermis) : pinkish blister,blanching, painful
c. 2nd degree (deep partial thickness – deep dermis) : whitish-yellowish blister, does not blanch, less pain
d. 3rd degree (full thickness) : white-leathery, less or no pain, not blanching
e. 4th degree

4. Treatment

a. Not severe/small area :
i. Outpatient
ii. cleanse with mild soap or antiseptic solution.
iii. Consider topical antibiotic like neomycin. Avoid Silver Sulfadizine (SSD) cream as it interferes with partial thickness wound healing.
iv. Synthetic occlusive dressing

b. Blisters : leave them unless large or affects joints. Aspiration preferred

c. IM ATT if 2nd degree or higher only

d. Severe

i. Admitted
ii. A**lgesia
iii. Small wounds : saline-soaked dressing
iv. Large wounds : sterile drapes
v. GI prophylaxis : pantoperazole
vi. No role of prophylaxis antibiotics

vii. Fluids given using below formula if > 15-20% (adults) or >10-15% (pediatrics) using Lactated Ringer’s (if not liver disease)
1. Parkland’s Formula (4mls/kg/TBSA of burns/24hour : ½ given over 8 hours and ½ over 16 hours). Or
2. Modified Brooke Formula (2mls/kg/TBSA of burns/24hour) ½ given over 8 hours and ½ over 16 hours)
3. In general 2-4mls/kg/TBSA of burns/24hour) ½ given over 8 hours and ½ over 16 hours)
4. Consider colloid if burns >40% adults or > 30% children

5. Burn can cause hypermetabolic response.
