Patient Present with the History of Obstructive Jaundice.
Periampullary Diverticulae Type 3 seen. Flat looking Papilla (type 1). Easy cannulation in 2 minutes. Selective deep CBD cannulation achieved with wire guided technique. Cholangiogram obtained after ultravist contrast injection which showed normal sized CBD and IHBD but short stricture with smooth tapering seen at distal end of CBD near ampullae. 10Fr x 10cm plastic billary stent placed across stricture in CBD. Free flow of bile seen.

Floppy looking Papilla (type 3). Difficult cannulation in 16 minutes requiring access Sphincterotomy. Selective deep CBD cannulation achieved with wire guided technique. Cholangiogram obtained after ultravist contrast injection which showed Normal sized CBD but extravasation of contrast before pacification of billary tree, near cystic duct stump, suggesting major CBD injury. 7Fr × 12 cm plastic billary stent placed in left ductal system across the leak for Billary drainage.
