Hints to avoid bile duct injury during lap chole…

Lap chole is easy abstain procedure ..
Don't ever underestimate it .
The most serious complication is bile duct injury .. followed by major arterial injury .
1-Surgeon 😷 should have an excellent knowledge about biliary tree 🌳 anatomy and its variation .
2-Use 4 ports technique .
3-Proper retraction of gall bladder fundus (toward Rt shoulder)
4-Retraction of hartman pouch lateral and caudal.
5-Start dissection above rouvier sulcus .
6-Acheive posterior dissection as well
7-Combin blunt and sharp dissection of calot triangle 🔻
8-Avoid excessive retraction and pulling GB up to avoid CBD tenting ..
9-Acheive critical view of safety .
10-Do not clip cystic duct unless you are 100% sure .
11-If there is doubt use intraop cholangiogram.
12-Mind the caterpillar hump of Rt hepatic artery .
13-Good hemostasis under vision
14-Use hook carefully in calot dissection
15-Avoid excessive use of cautery .
16-Handle cystic artery gently and avoid avulsion from RHA
17-Clip cystic duct close to GB
18-Dont go very low even in long cystic duct (milk cystic duct to push stones up )
18-If there is doubt take other collegue opinion .
19-Don't clip any duct considering it an accessory (do cholangiogram).
20-In cases of difficult anatomy try funds first technique.
21-Partial cholecystectomy or cholecystostomy tube are excellent option in difficult cases(don't feel shay).
take your time ( no body care about your record )
22-If bleeding occure don't panic use suction try to control bleeding under vision (no blind clipping )
23-If major bleeding don't hesitate to convert to open ..
24-Finally remember that Bile duct injury can occure to any patient 'With any surgeon 😷 ' and at any hospital 🏥…
25-Your aim in lap chole is to avoid bile duct injury (not to do cholecystectomy )
