Start of the weekend with another exciting surgical challenge.
Today managed a slightly distoangular impacted wisdom tooth featuring divergent roots 🦷. The real difficulty? The patient’s mouth opening was only ~20mm – making access and instrumentation quite demanding.

منہ میں بار بار سوجن؟ یہ صرف "پھوڑا" نہیں ہو سکتا!
کئی مریضوں کے منہ کے نیچے یا زبان کے نیچے ایک نرم شفاف گومڑ جیسی سوجن بنتی ہے، جسے عام زبان میں “Ranula” کہا جاتا ہے۔
یہ دراصل لعاب دہن (Salivary gland) کی نالی بند ہونے یا خراب ہونے سے بنتی ہے، جس میں تھوک جمع ہو کر تھیلی نما سوجن پیدا کرتا ہے۔

بہت سے لوگ یہ سمجھتے ہیں کہ دودھ کے دانت اہم نہیں ہوتے کیونکہ "ویسے بھی یہ گر جائیں گے"، لیکن یہ دانتوں کے متعلق سب سے بڑی غلط فہمیوں میں سے ایک ہے۔ دودھ کے دانت قدرتی طور پر جگہ بنانے والے (placeholders) کا کام کرتے ہیں اور مستقل دانتوں کو درست سمت اور ترتیب کے ساتھ اُبھرنے میں مدد دیتے ہیں۔ اگر ان دانتوں کی دیکھ بھال نہ کی جائے—چاہے وہ کیڑا لگنے، چوٹ لگنے یا قبل از وقت گرنے کی وجہ سے ہو—تو مستقل دانت غلط جگہ سے نکل سکتے ہیں، جس سے دانتوں کی بے ترتیبی، ہجوم یا حتیٰ کہ دانت پھنسنے (impaction) جیسے مسائل پیدا ہو سکتے ہیں۔

دودھ کے دانتوں کی جڑیں بالکل قریب ہی موجود مستقل دانتوں کے جراثیم (buds) کے ساتھ ہوتی ہیں۔ جب کیڑا لگا ہوا دانت علاج کے بغیر رہ جائے تو انفیکشن جڑوں سے آگے بڑھ کر براہِ راست ان مستقل دانتوں کو متاثر کر سکتا ہے۔ اس کے نتیجے میں نئے دانت پر داغ پڑ سکتے ہیں، تامچینی (enamel) میں نقص آ سکتا ہے یا دانت کی ساخت خراب ہو سکتی ہے۔ شدید صورتوں میں یہ انفیکشن ہڈی کو بھی نقصان پہنچا سکتا ہے، جس سے مستقبل میں دانت نکلنے کا عمل مزید پیچیدہ ہو جاتا ہے۔

لہٰذا دودھ کے دانتوں کی دیکھ بھال صرف بچپن میں درد یا انفیکشن سے بچنے کے لیے نہیں بلکہ مستقل دانتوں کے تحفظ کے لیے بھی ضروری ہے۔ باقاعدہ ڈینٹل چیک اپ، کیڑوں کا بروقت علاج اور گھریلو سطح پر درست نگہداشت ایک ایسا صحت مند ماحول فراہم کرتے ہیں جس میں مستقل دانت مضبوط اور صحیح جگہ پر اُبھر سکیں۔ بچپن میں ہونے والی غفلت کا اثر ہمیشہ قائم رہتا ہے—صحت مند دودھ کے دانت ہی زندگی بھر کی زبانی صحت (oral health) کی بنیاد ہیں۔
