World Prematurity Day. At this day we Pledge to make sure we are the safest care providers for our Preemies. We will try our best to keep each of the premature baby in the safest and near to natural environment. We will also make sure that every Preemies is provided with Kangaroo Mother Care (KMC) and expressed Breast Milk (EBM).

ہر سال یکم اگست سے 7 اگست تک دودھ پلانے کا ہفتہ منایا جاتا ہے۔ اس ہفتہ کو منانے کا مقصد دودھ پلانے کی اہمیت کو اجاگر کرنا ہے۔ ہمارا معاشرہ دودھ پلانے کے خلاف بعض خرافات سے بھرا ہوا ہے۔ کچھ لوگ کہتے ہیں کہ ماں کے دودھ کی کمی ہے جبکہ کچھ لوگ کہتے ہیں کہ ڈاکٹر صاحب پلیز ماں کے دودھ کے مائکرو آرگنزم کے ٹیسٹ کا مشورہ دیں۔ یہ سب بے بنیاد اور سائنسی طور پر غلط ثابت ہیں۔ براہ کرم آگاہ رہیں کہ کسی بھی نوزائیدہ کے لیے سب سے بہتر اور سب سے اہم ماں کا دودھ ہے۔

World Breastfeeding Week is celebrated every year across the world from August 1 to August 7. The global campaign is organized to raise awareness about breastfeeding and its benefits.
Below are mentioned some contraindications for breastfeeding.
Otherwise all babies parents should be encouraged for breastfeeding

Principles of transporting a sick newborn wether inside or outside of hospital.
Keep Normathermia.
Airway/Oxygenation.
Perfusion/Normotensive.
Maintain Normoglycemia.

In babies with HIE having multiorgan dysfunction with 2 or more systems ⬆️predicts death or moderate to severe brain injury on MRI.
Neurological (OR 13.5 [6.1-29.8] & renal dysfunction 11.2 [4.1-30.3] most strongly associated.

S.T.A.B.L.E Mnemonic of NICU.

A baby delivered at 31+5 weeks Gestational age who developed resp distress soon after birth was kept on Nasal CPAP. CXR Revealed grade II RDS. Curosurf Surfactant arranged and given via InSurE Method. In InSurE method baby is Intubated given Surfactant and soon extubated to Nasal CPAP. Currently the baby is doing well on NIV (Nasal CPAP)

Today we successfully extubated our first newborn who remained on Ventilatory Support for about 26 Hours due to Prematurity/RDS/Septic Shock. Currently baby is on indigenous CPAP and doing well. Indeed a Great Job by our team of Neonatal Nurses and Paediatric Residents.

Gadgets used in NICU

A token of appreciation from a Grandmother of twins who remained admitted in our unit and discharged in stable condition.
She brought 1.5 L of fresh Milk and 2.25 L of Desi Lassi after travelling for almost one and half hour from 25 KMs.
She was really happy and praising each staff and doctor of NICU.

Tiny baby in Singapore, just 7.5 ounces at birth, goes home after 13 months in hospital The new born baby girl, Kwek Yu Xuan, weighed barely half of the birth weight doctors had expected.

HYPOXIC ISCHEMIC ENCEPHALOPATHY (HIE)

CLINICALLY:
1. During labour: Fetal heart rate slows.
2. At delivery:
* Yellow meconium stained amniotic fluid
* Failure of spontaneous breathing
* Hypotonia
* Pallor, cyanosis, apnea
* Slow heart rate
* Unresponsive to stimulation
* Seizures refractory to anticonvulsants
* CHF
* Shock
* RDS
* GIT perforation
* Hematuria.

DIAGNOSTIC CRITERIA:
Any ( four ) of the following:
1. Evidence of fetal distress
2. Acute metabolic acidosis, pH < 7.2
3. Apgar at 5 min < 5 min
4. Initiation of respiration > 5 min
5. Features of HIE
6. Multi organ damage.

TREATMENT:
* Oxygenation.
* IV fluids (Maintain hydration 2/3rd of maintenance in case severe HIE).
* Aggressively manage shock.
* Maintenance of temperature.
* Maintenance of acid base balance and electrolyte imbalance.
* Treatment of infection by antibiotics.
* Neurprotective medicine like MgSo4, citicholine, melatonin etc.

Symptomatic treatment of:
– Hypoglycemia : 10% glucose 2 ml/kg
– Hypocalcemia : Calcium gluconate 10% 1 to 2 ml/kg
– Convulsions : Phenobarbitone 10-20 mg/kg loading dose ,
Maintenance dose 5 mg/kg/day.

* Brain cooling (if facilities exist).
