A token of appreciation from a Grandmother of twins who remained admitted in our unit and discharged in stable condition.
She brought 1.5 L of fresh Milk and 2.25 L of Desi Lassi after travelling for almost one and half hour from 25 KMs.
She was really happy and praising each staff and doctor of NICU.

1. During labour: Fetal heart rate slows.
2. At delivery:
* Yellow meconium stained amniotic fluid
* Failure of spontaneous breathing
* Hypotonia
* Pallor, cyanosis, apnea
* Slow heart rate
* Unresponsive to stimulation
* Seizures refractory to anticonvulsants
* Shock
* GIT perforation
* Hematuria.

Any ( four ) of the following:
1. Evidence of fetal distress
2. Acute metabolic acidosis, pH < 7.2
3. Apgar at 5 min < 5 min
4. Initiation of respiration > 5 min
5. Features of HIE
6. Multi organ damage.

* Oxygenation.
* IV fluids (Maintain hydration 2/3rd of maintenance in case severe HIE).
* Aggressively manage shock.
* Maintenance of temperature.
* Maintenance of acid base balance and electrolyte imbalance.
* Treatment of infection by antibiotics.
* Neurprotective medicine like MgSo4, citicholine, melatonin etc.

Symptomatic treatment of:
– Hypoglycemia : 10% glucose 2 ml/kg
– Hypocalcemia : Calcium gluconate 10% 1 to 2 ml/kg
– Convulsions : Phenobarbitone 10-20 mg/kg loading dose ,
Maintenance dose 5 mg/kg/day.

* Brain cooling (if facilities exist).

This is an X-Ray of 7 days old newborn with history of HIE Presented via ER with complaints of reluctance to feed coffee ground aspirats and abdominal distension.

Prolonged Fasting
Abdominal Distension
Gastric Residuals
Feed intolerance
We should think of NEC Necrotizing EnteroColitis.
In term baby if there is evidence of Perinatal Asphyxia still baby can develop NEC.
Human milk has a protective role in NEC.

Exchange Transfusion:
Baby with Total Bilirubin of 28 mg/dl
Exchange transfusion was done by Dr Tariq Afridi Resident Paediatrics succesfuly.


Interventions in Preterm babies for prevention of Bronchopulmonary dysplasia
A recently Published latest Systematic review and Meta Analysis.
These are:
1) Delivery room CPAP.
2) Early Rescue Surfactant
3) Early Inhaled Steroids
4) Avoidance of Intubation and Surfactant by LISA
5) Volume control ventilation as opposed to pressure controlled ventilation.


Baby boy remained admitted for 1 month in our unit with MAS PPHN NNS later on had Post COVID MIS-N. He fought all the challenges and graduated successfully. Real happiness for our team and parents of the baby.


32 weeker twins discharge after 16days of stay in nicu..goes thru tough time (sepsis,jaundice,apneac spells)..
Salute to mother of twins who brought expressed breast milk for twins..
Discharge on Full ng tube feed and advice to follow after 1week..
