FOOT DROP

Foot drop is the inability to dorsiflex your ankle or having difficulty in lifting your forefoot. This normally occurs following weakness in any of the ankle dorsiflexors, usually the tibialis anterior muscle.

The weakness occurs as a result of an injury to the main nerve that gives innervation to the tibialis anterior muscle called common peroneal nerve.

The common peroneal nerve is one of two main branches of the sciatic nerve. The sciatic nerve is a large nerve which takes its roots from L4-S3 spinal roots and travels down the thigh before dividing into two main branches, which one of them forms the common peroneal nerve. The common peroneal nerve further divides into deep and superficial nerves.

The deep peroneal nerve gives innervation to the tibialis anterior muscle, which is the primary mover of the ankle into dorsiflexion.

Injury to this nerve will result to weakness in the tibialis anterior muscle and this will result to a dropped foot.

This means that, there will be an unopposed force from the ankle dorsiflexors due to the weakness, and will cause the foot to remain plantarflexed.
When this is not corrected, the patient gait will be badly affected.

CAUSES/RISK FACTORS:
The main cause of foot drop can be as result of injury or irritation of the deep peroneal nerve. As the deep peroneal nerve is one of the branches of the sciatic nerve, having problem with the sciatic nerve predisposes one’s chances for foot drop.

Conditions that create sciatic pressure such as lumbar disc herniation, spondylolisthesis,and spinal stenosis, as well as infection, medical problems like tumor or stroke, neurological disorders like amyotrophic lateral sclerosis, GBS, Parkinson’s disease and muscular dystrophy may equally pose a higher risk for foot drop. In the same way, foot drop may occur following irritation of the peroneal nerve in hip and knee replacement surgeries.

SIGNS AND SYMPTOMS:

Early signs of foot drop include the following;
1. Numbness on the top part of the foot
2. Trouble holding onto footwear, especially flip-flops or shoes without back
3. Weakness lifting the foot at the ankle

The latter can be the main reason for a stepping gait pattern in most patients with foot drop, which is very abnormal.

TAILBONE PAIN

Tailbone pain, also called coccydynia, is pain in and around the small triangular bone at the very bottom of your spinal column, above the cleft of your buttocks known as the coccyx.

The term coccyx comes from the Greek word for “cuckoo” as it resembles a bird’s beak with the tip pointed down. “Dynia” means “pain,” and so “coccydynia” literally means “pain of the coccyx.” And because the bone corresponds to the location of an animal’s tail, it’s called the tailbone in common terms.

Anatomy of the coccyx:
Your coccyx is made up of three to five fused vertebrae (bones). It lies beneath the sacrum, a bone structure at the base of your spine. Several tendons, muscles and ligaments connect to it.

Both the coccyx and the ischial tuberosities (two bones that make up the bottom of your pelvis) bear your weight when you sit down. Two-thirds of adults have a coccyx that curves a bit instead of pointing straight down, but one that is curved too far is abnormal and, therefore, painful.Tailbone pain (coccydynia) can make everyday tasks uncomfortable at best, and unbearable at worst.

Tailbone pain ranges from a dull ache to a fierce stab. It can last for weeks, months or sometimes longer. There are three types of events that cause tailbone pain:

External Trauma: A bruised, broken or dislocated coccyx caused by a fall.
Internal Trauma: Trauma caused by a difficult childbirth or from sitting on a narrow or hard surface for too long.
Others: Infection, abscess and tumors.

Interestingly, for one-third of those with coccydynia, the cause is unknown. Women are five times more likely than men to develop coccydynia. Adults and adolescents get it more often than children. Obese persons are three times more susceptible than those at the ideal weight according to the BMI (Body Mass Index) scale. You’re also more vulnerable if you lose weight too quickly.

Symptoms:
The symptoms of coccydynia include:
a. Achy or piercing pain in the tailbone.
b. More severe pain when changing from sitting to standing up.
c. More severe pain when sitting for long periods of time.
d. Pain during bowel movements.
e. Pain during s*x.
Other related symptoms that may occur with coccydynia include:

•Depression.
•Anxiety.
•Poor sleep.
•Painful buttocks.
•Back pain.

Groin Tendinopathy

💢 Groin tendinopathy is a condition characterized by pain and inflammation in the tendons located in the groin area.

It is commonly seen in athletes who participate in sports that involve repetitive movements, such as running, jumping, or kicking.

The condition typically affects the adductor muscles, which are responsible for bringing the legs together.

The exact cause of groin tendinopathy is not fully understood, but it is believed to result from overuse or repetitive stress on the tendons.

This can lead to microtears in the tendon tissue and subsequent inflammation.

Other contributing factors may include muscle imbalances, poor biomechanics, inadequate warm-up or stretching, and previous groin injuries.

The primary symptom of groin tendinopathy is pain, which is usually localized to the groin area.

The pain may worsen during activities that involve adduction or resisted hip movements, and it may also be present at rest. In some cases, swelling and tenderness may be observed in the affected area.

Diagnosis of groin tendinopathy typically involves a thorough physical examination, where the doctor will assess the range of motion, strength, and tenderness in the groin region.

Imaging tests such as ultrasound or MRI may be ordered to evaluate the extent of the injury and to rule out other possible causes of pain.

Treatment for groin tendinopathy usually involves a combination of conservative measures aimed at reducing pain and promoting healing.

This may include rest and modification of activities to avoid exacerbating the condition, along with the use of nonsteroidal anti-inflammatory drugs (NSAIDs) to reduce pain and inflammation.

Physical therapy may be recommended to strengthen the surrounding muscles, improve flexibility, and correct any biomechanical issues.

Surgery is typically reserved for cases that do not respond to conservative treatments and involve significant tendon damage.

Different kinds of spinal problems and their explanation

1. Herniated Disc

A herniated disc is characterized by having the inner content of the intervertebral disc pushed out of place. This forms a hard substance that is capable of pinching on nearby spinal nerves or reduce the spaces of spinal foramen. It’s one of the major causes of sciatica in most of the cases.

2. Disc Bulging

A disc bulging occurs when the intervertebral disc bulges out like balloon. This can occur following an uneven loading on the spine where the compressive force presses on the disc from one side and in effect, causes the disc to bulge out.
Unlike disc herniation, the inner content of the disc remains intact in the case of disc bulging. It is also a common condition of the spine that accounts for most cases of back pain and sciatica

3. Disc Degeneration

This is age-related condition of the spine. It’s characterized by wear and tear of the intervertebral disc. It’s the osteoarthritis of the spine, often termed as spondylosis. The disc in between the vertebrae wears out either partially or completely. This tends to collapse the two vertebrae that the disc finds itself in between.
It’s the major cause of back pain in most elderly patients.

4. Osteophyte growth or bony spurs

This is the growth of extra bones around the vertebrae bones. It’s often a complication of disc degeneration. The extra growth of bones can piers through surrounding structures like spinal nerves, ligament, tendons and even muscles. It’s one of the problems that can result to nerve root compression to account for radicular pains.

Most of these conditions can be potential cause for back pains and it’s the more reason why your therapist or doctor requests for images of scans like MRI, CT Scan or X-ray once you come with back pain complaints.

FLAT FEET (PES PLANUS)

Flat feet is a condition in which the normal arches on the inside of the feet are low or missing.This gives the foot a flat appearance with the entire sole of the foot contacting the ground.

There are two main types of flat feet:
1. Flexible
2. Rigid

Flexible flat feet are the most common. A flexible flat foot has the ability to form an arch but the arch flattens when standing.

A rigid flat foot, on the other hand, cannot form an arch.

Flat foot versus normal foot
Flexible flat feet often respond well to exercises designed to increase the height and stability of the arch. While exercise alone may not change the arch shape for other types of flat feet, it can still be helpful for managing pain or improving overall foot function in some cases.
