A kid with chronic osteomylitis, a story of reconstruction..
Magic of Masquelte technique & Ilizarov fixation..
He was presented with exposed proximal tibia with discharge of moderate amount of pus & a year of bed ridden & multiple debridments..

Management :
* debridment + sequesterectomy & bone cement loaded antibiotic spacer
* 6 weeks later,, wound healed, inflammatory markers normalized,.. Planed for 2nd stage =
Bone graft & Ilizarov fixation
After 7 months he was ready to remove the frame & walking happily independent.

Wissam 6 years boy a victim of explosive missiles in Khartoum war..
Presented 1 year ago with proximal tibia open fracture , bone loss and soft tissue defect anterior knee..
Underwent debridement and ex fix the day of trauma and referred.
Presented to us with bad skin condition and loose ex fix
Which have done.... debridement ,sent to plastic surgeon for soft tissue cover , then 2nd debridement + bone cement spacer ...
After 8 weeks inflammatory markers normalised & skin healed but there was valgus deformity & proximal tibia growth plate was disrupted ...
For now i did deformity correction & iliac crest bone graft...
Good news patient started to walk after a year of bed ridden and back to school ...

- burned dorsal skin at exit site with extensors tendons cut
Fortunately normal NV.
Management in 3 sections
1st debridement + K. wire fixation to keep the alignment
2nd plastic surgeon put a wonderful skin graft after serial of dressings
3rd ORIF of radial + non_ vascularized fibular graft.
Later for tendons transfer .....
