✨ Understanding Embryo Scoring in IVF ✨
When you see embryo scores like “5AA” or “3BC,” what does it actually mean? 🤔

🔹 Numbers range from 3 to 6, representing the stage of blastocyst development. For example, 6 means a fully hatched blastocyst. Embryos graded between 3 and 6 all have the potential to implant.
🔹 Second letter (A–C) shows the quality of the inner cell mass (ICM), which develops into the baby.
🔹 Third letter (A–C) shows the quality of the trophectoderm (TE), which develops into the placenta.

But here’s the important part:
👉 Embryo grading is only a visual assessment by embryologists under a microscope. It doesn’t guarantee success or failure. High scores may increase the chances of implantation, but even low-scoring embryos can lead to healthy pregnancies ❤️.

🌱 If faced with a low-scoring embryo:
1. Don’t lose hope — some lower-grade embryos still have the ability to implant and can grow into healthy babies.
2. Talk to your doctor about PGT-A testing for deeper insight.
3. Support your body with healthy habits, rest, and stress management.

Last summer, Ms. Yin came to NUWA Fertility Center Taichung Clinic for the first time. She was extremely anxious, having just received her AMH results from an outside lab a week prior—at not even 32 years old, her AMH was only 1.05. Dr. Huang comforted her, sharing that her own AMH at 32 had been just over 1, yet ten years later, she had three children! 🤰 Dr. Huang encouraged her: “Knowing your egg reserve allows you to plan ahead. If you hope to become a mother and it’s financially possible, freezing eggs early is wise.”

Over the next four months, Ms. Yin took fertility supplements, exercised, and adjusted her lifestyle. By year-end, her egg freezing treatment began. At the start of her cycle, she insisted on checking AMH again at our clinic; the result was 1.03. Dr. Huang explained: “AMH is about 80% accurate and lifestyle changes or stress relief can cause minor shifts—either small increases or slower declines.”

That cycle, seven mature eggs (MII) were frozen. Six months later, with AMH dropping to 0.28, we started a second round of egg freezing. 💪

Diligent as always, Ms. Yin researched before each visit. Before her second retrieval, she asked: “Is ovarian PRP really effective?” PRP (platelet-rich plasma) is concentrated plasma from one’s blood, rich in growth factors and cytokines, which can help tissue repair and cell growth. As an add-on therapy for ovarian rejuvenation, PRP has been used for over a decade, but most studies are small or show wide individual differences. 🤔 Still, research suggests that while PRP may not significantly raise AMH, it could increase mature eggs and blastocyst formation over 2–3 months.

During her second cycle, Ms. Yin opted for PRP. After egg retrieval, she rested for two months, continuing supplements and traveling monthly from Yunlin to Taichung for blood tests: AMH rose from 0.537 (June 1) → 0.824 (July 2) → 1.06 (August 1). In August, another retrieval yielded seven more mature eggs.

Over the year, including four retrievals (one double stimulation cycle), a total of 18 usable eggs were collected. 💖She had completed this stage of her journey, carrying reassurance and hope for the path ahead. 🤗

🌸 What to Expect When Freezing Your Eggs in Taiwan
Thinking about egg freezing but not sure where to start, or which clinic to choose? Here’s what you should know!

❄️ Why Egg Freezing Matters
A woman’s fertility starts to decline gradually after age 35, and more rapidly after 38. With modern technology like vitrification, egg freezing allows you to preserve your fertility and keep your future family options open.

✈️ How Egg Freezing Works in Taiwan
You may need to visit Taiwan 2–3 times for the egg freezing process. Alternatively, you can choose to stay for about 2 weeks and complete the entire treatment in one visit. Here’s a step-by-step overview:

1️⃣ Initial Consultation & Medication
Book your appointment between Days 2–4 of your period. You'll receive:
- Hormone tests (FSH/LH/E2/P4/AMH)
- Transvaginal ultrasound
- Ovulation induction medication or injections

2️⃣ Monitoring & Trigger Shot
Take daily injections for 8–12 days to stimulate follicle growth. Blood tests and ultrasounds to track progress and medication response.
Once follicles are ready, a trigger shot will be administered 36 hours before retrieval.

3️⃣ Egg Retrieval
Eggs are retrieved under general anesthesia. If embryo freezing is preferred, s***m collection can be arranged.

Please note: Egg freezing typically takes about 2 weeks starting from the first day of your menstrual cycle. The doctor will be able to personalized the treatment plan based on your availability.

Doctors emphasize that if you are diagnosed with chocolate cysts (endometriomas) at a younger age, it’s important to discuss your pregnancy plan early on. This can help avoid unnecessary delays or undergoing multiple surgeries that may impact your ovarian reserve.

So what should you know about ovarian cysts?

🧡 What Are Ovarian Cysts? Here’s What You Should Know 🧡
Ever heard of ovarian cysts but not quite sure what they are or if you should worry? Let’s break it down 👇

🩺 Ovarian Cysts 101
Ovarian cysts are fluid-filled or semi-solid sacs that develop on or inside the ovaries, and yes, they’re super common. They come in two main types:

1️⃣ Functional Cysts – Totally Normal!
These develop as part of your regular ovulation cycle (like follicular or corpus luteum cysts).
✔ Usually harmless
✔ Often go away on their own in 2–3 months
✔ Don’t usually affect fertility

2️⃣ Pathological Cysts – Need More Attention
These result from abnormal cell growth and include:
🔸 Endometriomas (aka chocolate cysts)
🔸 PCOS (Polycystic O***y Syndrome)
🔸 Dermoid cysts & cystadenomas
🔸 Ovarian cancer (rare but serious)

Some of these may affect fertility or require surgery, especially if they grow large or cause symptoms.

🚨 Signs You Shouldn’t Ignore:
Most cysts are silent, but when symptoms appear, they might include:
🔹 Dull pelvic or lower abdominal pain
🔹 Bloating or pressure in the belly
🔹 Pain during s*x
🔹 Irregular periods
🔹 Frequent or difficult urination

Different types of Pathological cysts:

🔸 Endometriomas (Chocolate Cysts)
These can lead to chronic pelvic inflammation, which may negatively impact egg quality and implantation. In more severe cases, they may cause premature ovarian insufficiency, reducing the chances of conception.

🔸 Polycystic O***y Syndrome (PCOS)
A hormonal imbalance that disrupts ovulation, often leading to irregular or absent ovulation. PCOS is one of the most common causes of infertility due to ovulatory dysfunction.

🔸 Dermoid Cysts and Cystadenomas
If these cysts grow too large (typically over 5 cm), they can increase the risk of ovarian torsion or rupture. Ovarian torsion may cut off the blood supply to the o***y, potentially leading to tissue death, which can significantly affect fertility.

🔸Ovarian Cancer
In most cases, surgery is required to remove the affected o***y. If you are planning to have children in the future, it's crucial to discuss fertility-preserving options, such as ovarian preservation or egg freezing, with your doctor before treatment.

Over the years of clinical practice, Dr. Lin has also been a teacher—guiding medical students and young minds in the life sciences. 🧑‍🏫 Perhaps that’s why there’s always a hint of “teacher” in the way he speaks.

She, nearly 36 years old, came all the way from Hong Kong and paid close attention to every detail of her treatment—just like a diligent student. 📚

Due to a previous ovarian surgery, her ovarian reserve had clearly declined, with an AMH of only 1.27 ng/mL. She and her husband hoped to have two children in the future, and they were concerned that egg retrieval might be even more difficult for the second child. So, together with Dr. Lin, they formed a strategy: to first prepare enough embryos in advance. 🧬

With well-planned medication and scheduling, they performed two egg retrievals, resulting in 14 blastocysts. All were sent for PGS testing, and ultimately, they obtained 4 normal embryos. The first embryo transfer led to successful implantation and development—and she graduated from the program smoothly! 🎓👶

On the day of her graduation, they smiled and asked, “Dr. Lin, does this mean we’ve achieved 1-1-1?”

Their expressions were like students eagerly awaiting a teacher’s response.

Dr. Lin smiled and replied, “Strictly speaking, not quite 1-1-1, because we did two egg retrievals. But this was to prepare for the second baby—it was completely reasonable and wise. And it was your dedication and commitment that made it possible for the very first single embryo transfer to succeed—just as impressive as a 1-1-1!” 🌟

The 1-1-1 technique, developed by Dr. Lin, refers to 1 successful egg retrieval, 1 successful embryo transfer, and 1 more successful transfer in the future—for two healthy pregnancies. 👶👶

🔬 What Is PGT-A and Who Should Consider It?
PGT-A (Preimplantation Genetic Testing for Aneuploidy) is a test done on embryos during IVF to check if they have the correct number of chromosomes.
✅ It helps identify genetically normal embryos before transfer.
✅ It can screen for Down sydnrome, Turner syndrome, Klinefelter Syndrome, Edwards syndrome, Patau syndrome, etc.

Who may benefit from PGT-A?
- Women over 35
- Those with Recurrent pregnancy loss
- Multiple failed IVF cycles
- Known chromosomal issue
- Those who opt for single embryo implantation to increase pregnancy rate

📈 How Does PGT-A Improve IVF Success?
- Increases chance of transferring a healthy embryo : embryos with a normal chromosome count (also called euploid embryos) are more likely to develop properly, attach to the uterine lining, and result in a healthy pregnancy.
- Lowers risk of miscarriage due to implanting Poor-quality embryo : By screening out embryos with chromosomal abnormalities, PGT-A reduces the risk of early pregnancy loss due to it’s inability to develop properly.
- Reduces the number of IVF cycles needed : By identifying the most viable embryos early on, PGT-A can help achieve pregnancy faster, potentially reducing the number of IVF cycles required.

❓ FAQs About PGT-A
Q1: What differentiates embryo scoring from PGT-A testing?
A: Embryo scoring looks at appearance under the microscope; PGT-A looks at the genetic content.

Q2: Does PGT-A harm the embryo?
A: Although there is a risk, only a few cells are gently taken from the embryo, and the process is safe when done by experienced professionals.

Q3: Once I’ve done PGS screening and successfully conceived, is it 100% guaranteed that my baby will be healthy?
A: It is not 100% guaranteed! Whether it’s done through PGS or PGD testing, not all chromosomal abnormalities can be successfully screened, so maintaining a healthy condition throughout pregnancy is still imperative to a successful pregnancy!
