BASIC INFORMATION TO BE PREPARED FOR YOUR URGENT CARE VISIT
We are a walk-in clinic, or you are welcome to schedule online at our website:

What you will need to bring:
• A form of identification: Driver’s license, state issued photo ID, passport,
military ID.
• Proof of insurance: Bring your insurance card and subscriber information so
the claims can be filed correctly.
• Contact information: Be prepared to provide your personal contact
information as well as emergency contact information.
• Forms to be filled out: Bring any paperwork that needs to be filled out and
signed for schools, employers, etc.
• Questions you have: Have a list of questions you want to be sure to ask at
your examination. This way you will get all the answers you need.
• Method of payment: We accept most major insurances. We will verify your
insurance and collect your urgent care co pay if there is any.
Health Information:
• Physician information: Be able to provide your primary care physician
information for any provider-to-provider contact and medical records.
• Health history: Know your personal health and family health history.
• Medications: A list of all prescriptions and over-the-counter
medications that you are currently taking.
• Allergies: Be able to provide a list of any known allergies.
• Past medical procedures: List all surgeries or medical procedures that
you have had in the past.
• Medical conditions: List all medical diagnoses such as asthma, blood
disorders, diabetes, etc.

After hours: We are open 7 days a week from 8:30am-8:45pm. We are available if
you’re having health issues that can’t wait.
Out of town patients: If you are not a resident of New Mexico and you are having
health issues, Doctors Urgent Care provides fast, quality healthcare services. You
could be seen and treated without your primary care provider.

No Primary Care Doctor? Your medical needs do not need to be urgent for you to
visit an urgent care. We provide medical attention for those in need.
