The Neurosurgical Society of the Virginias concluded a spectacular annual meeting at the Salamander Middleburg Resort this past weekend. We had a large number of impressive lectures and poster presentations by attending neurosurgeons, residents and medical students from Virginia, West Virginia, Maryland, Washington DC and North Carolina. We were particularly honored by the presence of Dr. Steven Kalkanis MD as this year's honored guest.

Congratulations to the society's president, Dr. Mateo Ziu, MD, MBA, FAANS, and to the secretary, Dr. Scott Graham, for running a tremendously successful meeting.

The alleged killer of the United Healthcare CEO was caught yesterday. Ongoing investigation is slowly revealing a triple tragedy. The most recent is a senseless and heinous murder. Overarching that is a challenging health insurance environment based in greed and focused on maximizing profit rather than oriented toward patient care. But the third is a tremendous amount of pain and suffering from a spine condition and a failed spine surgery.

Below is the postop X-ray of Luigi Mangione, the alleged criminal. He had an L5-S1 grade 1 spondylolisthesis, or malalignment of two bones in the lower back. In this case, the condition appears to have been treated through an instrumented fusion. Reportedly, Luigi’s failed surgery and chronic pain turned him “crazy”; from a valedictorian and an Ivy League graduate to a killer.

The next few weeks, as well as the 26-year-old’s 262-word handwritten manifesto, will reveal a lot more about Mangione’s struggles with the medical insurance world. But equally important will be our understanding of his condition and of the reasons why his surgery failed. Whether he was misdiagnosed at the outset, whether the choice or the ex*****on of the surgery was inadequate, or whether he needed more support during the postoperative course are all questions that I hope time will help us answer so we can become better spine surgeons. As for today, let us learn as physicians to never underestimate how pain can take a toll on our patients.
