Saturday, I found my PT school bible (my binder from gross anatomy). As a physical therapist, I’ve seen firsthand that true healing requires looking at the whole person. I’ve evolved my practice to truly center on a holistic approach reflected in this new name and serene aesthetic.

Geez Louise! This is SO long, but Im just too excited to not share all of it. The twists & turns, hills & valleys of breast cancer recovery! The crap I’m learning though ~is rich! & I plan to share it too. This one is about running & not primarily related to breast cancer, but an old injury!

A soon future addition will be on “swimming & arm recovery after mastectomy & radiation.”

Sorry for the spotty appearances on this page, but I am still in treatment until late this upcoming fall, so in the meantime, I’m going to share personal insights & learnings related to health & rehab in the realm of ‘breast cancer AND returning to full & vibrant health & athletics!!!

For the PT’s here, please ask any questions, bc it’s been very interesting trying to figure out what was injured.

I’d like to give a shoutout to some colleagues (Anna Faulkner, Gayanne Grossman & Maggie Eyvazzadeh & a very special guy (my husband Tim) for helping me out of a chronic hip pain situation that prevented me from achieving a running gait for over 10 years. Over a decade ago, someone decided to stretch my leg into flexion abduction (up & out) fast & furious (I had given permission for passive stretching, just not for it to be a one rep stretch that was intending to undo 10 yrs of tightness.)

There was immediate immense pain, a ripping sensation, I screamed, pulled my leg back to myself & had pain for 6+ months just standing & walking. I Should’ve sought out an MRI, but was embarrassed. Was not my fault, & I would respond differently now, in hindsight, especially with the degree of injury that was caused. This person should’ve been told at the time. Now it’s too late for them to reasonably accept responsibility.

As recent as last year, we speculated a possible traumatic posterior labral tear of the hip. There was definitely some nerve restrictions in these later years because when I worked on it, I had nerve pain the next day or so.

During these later years, the reproducible pain was only when attempting to run. So that being quite a deterent to run, I have become sorely out of running shape! I have been working (both gently & sometimes a tad more aggressively) on scar tissue releases & stretches in my groin and posterior (rear) hip on my own for years on/off, but being that it was hard to reach, I could only get so far.

The amount of scar tissue in these personal areas was significant & finally required the assistance of colleagues who are comfortable working in private spaces, & thats how these colleagues & friends were carefully selected! It even included some pelvic floor work with my colleague & friend Anna, and my husband. PSA Ladies: if both parties are willing, husbands can be taught how to assist & perform pelvic floor release work. If either of you are not comfortable or interested in this option, that is fine too, as your PFM PT can handle it.❤️

Well… initially, I really couldn’t tell if it was groin (adductor) or back of thigh (medial, inside hamstring) or both. I do believe both were part of it, along with some deeper muscles (pectinius probably). As time went on, small rotators got in on the ‘compensatory weakness/spasm’ train. As anyone with a chronic hip issue knows, the other hip muscles that are not initially involved with the problem, will want in on the dysfunction issue, if the original problem is not resolved in a timely manner!

My main symptom/problem was this tethering pull that prevented my right leg from advancing forward into a full stride & the pain went from groin (front & back) to the outer hip, with pain increasing with time & distance ran.

As things were resolving in the scar tissue, with the pain not better, & I still couldn’t get into a running gait, I was suspecting a deeply located, small hip external rotator (obturator externus) as a prime suspect in the specific thing that pulled my leg back & caused pain.

I am elated that it did seem to be the case bc upon its release last week, the degree of that pull & pain is significantly less, dare I say gone!?! I cannot share how psyched I am to be free of this thing that wouldn’t let me run!!!

So now even though I am dealing with running on weak legs, a weakened & repaired abdominal wall & running after 10+ years of not running while going through chemotherapy the past 13 months, it is going to be ok. Because my abdominal wall, as weak as it is, holds me together better, than when it was partially apart the past 16 yrs & my breath control will improve.

But it maybe was not a good idea to do my first time trial in this heat! 😬

Anyway, 3x the past 2 weeks I went on the AlterG treadmill (with only 55% of my body weight, 45% of my weight reduced).

DAY 1: 1 min (lightheaded), then 2 min )less lightheaded running 6.0 pace. Walk before/after running.

DAY 2: 4 min, then 3 minutes same pace, not lightheaded, stomach muscles sore & not quite sure what’s going on!

DAY 3: 1 mile at 6.0 pace, then another mile at 9:05 pace. Belly muscles very sore… Remember 45% of my weight was unweighted but it felt SO GOOD to really run! Next day, Sat, my back & legs were not happy!

So instead of my first full weighted run to be around the block (.37 of a mile, 9 laps = 5k), I was quite zealous & decided to map out a 1 mile a time trial in this heat!

Ha! I made it 2 mini blocks (.12 mile; 17 = 1 mile), then had to walk 3 mini blocks, & repeated that 2:3 run:walk pattern for the rest of the mile finishing in 13:36 minutes, so that’s that.

LOTS of room for improvement, but the pulling WAS NOT THERE!!! I ran 1 mini block on the return walk home, then felt full effects of heat, lightheaded, possible continued poor body temperature regulation, headache, which stuck with me for several hours!

Im super grateful for the opportunity to build stamina on the alterG treadmill which allows for the exact task in a less demanding manner!

So ~ Im planning to run the entire Women's 5K Classic this year (Oct 5) (last year we walked it with 5-6 mini bouts of jogging for a 52 min 5K)! I’ve got 3 tiers of goals for 2024! Something hit me hard in November! Felt truly like a Mack truck, I kid you not!

2 weeks, ONLY 2 weeks into my recovery …basically NOT recovered at all, after a bilateral mastectomy & abdominoplasty that required an 8x10 piece of mesh to be sewn throughout the surface of my re**us abdominus, only 2 weeks later, I could move my body WAY BETTER than I had been able to move for the past SIXTEEN yrs, since my big, nearly full-term twins occupied my belly, separating my left & right re**us abdominus, ~all 12.5 lbs of them, plus the other stuff in there!!! I got used to something that bad out of necessity!

Not a pretty description, I get it, but what women go through in female health things like pregnancy, childbirth, dysmenorrhea (painful periods), endometriosis, dyspareunia (painful s*x), mastectomy, laparoscopies, speculum exams, etc, with generally little complaint, is not pretty in reality! If you report a problem sometimes you're a complainer or maybe if it’s really bad, you become emotional!?!
Oh dear! We can’t have that!

Research has shown that for incontinence, men demand/receive help: within a week, women: 20 years! After my first child, I reported it at the 1 month follow up, was told it was normal, & given NO recommendations!

Luckily for me, I was ‘privileged’ to be an educated consumer & also a PT, so I asked for a script for PT! This begs the question… Within those ‘20’ years of women not being helped, how many times were their reports of leaking ignored or minimized?

What about women …who are not educated?, …Whose english is their second language? …who are members of marginalized communities related to skin color, weight, disability status? Anyone asking or listening?

My repair in the absence of mastectomy, would have been ‘cosmetic’ & not covered by insurance. If it had been done sooner, which I admittedly was quite reluctant to do, my muscles would have still had their strength; mesh may not have been needed. But I started to look into it finally, & got no where!

My recti have been in the wrong place for over 16 years, impairing my stability & causing pain throughout my spine, especially my neck & although I reduced my abdominal separation by more than half, they consistently re-opened some, with any higher level work, such as burpees, mtn climbers, sprint work, planks, hanging, pull ups, over-unders, and many other body weight movement exercises, pilates & even yoga!

And so,
Gaining fitness = greater loss of stability/more pain
How does one choose?

BTW, It is very possible to reconnect diastasis in many situations through specific motor control training among the muscles of the abdomen, diaphragm, pelvic floor & in gaining awareness & control of the pressures with the cavities of our body (thoracic, abdominal, pelvic cavities)! For me, at 7 cm, that was a severe situation, & other considerations should have been an option, but were not!

In the end for me, it’s all good, bc if I had the procedure earlier, the option I chose for breast reconstruction would not have been available anymore. But this post isn’t actually about me. Its about addressing women’s health needs with equity! I just happen to have a really good example.

When one tears a quad or rotator cuff muscle that moves a leg or arm, PT is recommended nearly right away, surgery is at least considered & covered if needed. Damage that is incurred upon a woman's body in pregnancy, even though she likely chose to become pregnant, is affecting movement of the whole body, is often much more limiting in nature, is virtually ignored, & surgery is denied bc it’s cosmetic!

Pregnant women should have an automatic referral to PT in pregnancy, in the immediate postpartum either at hospital or home, with determined follow up as needed! I have been told that the automatic referrals for PTs/physios are a standard of care in many European countries & Australia! Let's go US!

Shoulder issues? Nerve pain in the arm?

If your body is dealing with an injury that involves the nervous system, you may get messages of the shooting, stabbing, burning type… The information can be confused by feelings of tightness, aching, cold, swelling.

As a therapist, keep in mind that our body’s vasculature runs alongside the nervous system, and restrictions on one system could actually impact connection & function of both the nervous and vascular systems.

Maybe your injury seemed to be a localized wrist strain with local distal (wrist/hand) nerve involvement? However, shortly there after, pain emerges elsewhere (further up the arm)?

Our nervous system is so sensitive and it is critical to address the full course of the nerve(s), all the way back to the plexus, brain stem, and brain, & while your at it, the vasculature too!

AND also while your at it, all if the supporting musculature in the area. The latissimus dorsi, ‘lat’ for short, is a superficial & wide back muscle that we work in the ‘lat pull down’ machine. It’s fascia connects to your pelvis and it traverses your entire back. Guess what? It also attaches at the shoulder and internally rotates the shoulder! Wow!

Today, my patient’s ‘lat’ was unhappy trying to support all of the things going on with the muscles, nerves, arteries, & veins in her neck, shoulder, & arm. A little love and support to the ‘lat’ went a long way.

This muscle was speaking up bc it was trying to do something it’s not meant to do. I call these muscles overworked and underpaid!

Who here can guess what I consider the 2 most overworked & underpaid muscles in the body? Patients feel free to share if you remember one or both. Abbreviations are allowed!!!

Hello, here’s a little video offering a brief update on what has been going on this summer with me. I was diagnosed with stage three invasive ductal carcinoma, the most common type (blessing 1: tons of research).

Gratefully, I found (Holy Spirit lead me to…) a treatment team, who is part of a clinical trial, which I qualified for, and which offered a treatment plan that does not blow up my functionality!!!

I am very grateful to have been able to continue treat my clients, and have found that my time with my clients has been a wonderful opportunity to continue to just be me. I understand that I am a patient too, but I don’t actually feel sick and I have yet to really feel sick, so I’m fine not really feeling like a patient!

I will go through this process, do what I need to do, and presume to resume my pre-cancer life, God willing cancer-free, with many new learnings understandings about myself, and what will be many rich takeaways and lessons. I have already well learned that the hardest things bring the best kind of deep growth & so this lesson does not need time to get into my heart.

I am definitely very nervous about some of the implications of the treatments to come. I will face those as they come. I do not plan to evade all of the negatives; I will feel them, process them, share what is applicable, but will find a way to make lemonade & I actually have been known to eat lemons too (with a sugar stick!)!

💕Trust in your healthcare provider is earned when you as a consumer feel connected to your providers approach, their interpretation of your circumstances (& it compliments what you feel & know about your circumstances), & when the provider’s skills & philosophy of care facilitate your processing, your healing, & your progress. Trust is not owed to a provider when true connectedness & mutual understanding are not met, regardless of education, status, & accolades!

I am opening up some new treatment slots, so I'm reaching out to my people to help other people find me! Before, I was nervous that my schedule would fill up and I didn't want a wait list, so I stayed quiet, but that is a little silly!
Because I am bound by HIPPA privacy rules and just to be trustworthy, I do not share stories, so some people think that I am super private. If I shared a unique story (& there are many), some of my patients could say, "she's totally talking about me!" & that might be weird for them. So please feel free to share anything that is not breaking your personal boundary of privacy! I treat a huge variety of conditions, and my pelvic floor training and advanced soft tissue training has helped me make big strides for people dealing with long standing chronic conditions like frozen shoulder, plantar fasciitis, etc. Thanks!!!

Prayers for all who are oppressed, in particular the women who are discussed in this story. It is truly heartbreaking, tragic, & scary. As a woman who specializes in women’s health & pelvic rehab, I find it incredibly horrifying that there may be no women, not enough women, or decreasing numbers of trained female providers for this type of care, if it is provided at all in this current state. Even in our country, exists significant inadequacies & disproportionate outcomes in women’s health care, but it is in the presence of a free society where we have the opportunity to learn & teach & work to make improvements without a threat to our personal safety.

The killing of the young former politician comes as women's rights across Afghanistan are chipped away by the Taliban. Police have not yet been able to name a suspect or a motive.
