An innovative and affordable home membership program designed to meet the needs of active seniors

Pathstones by Phoebe is a continuing care at home program that provides another option in retirement living. We combine key features of living in a continuing care retirement community; long term care insurance and care management and deliver them to you in your home. We use our knowledge and expertise to help you take control of your future. We get to know each member very personally and together design a plan to maintain and promote wellness. We also get to know your preferences in the event a health crisis should occur. As a result, if you experience an injury or illness, we are able to support you every step of the way. Our wellness coordinator will serve as an advocate for you and your family to facilitate your care and determine next steps. Additionally, depending on the plan selected, you will receive a daily monetary benefit toward the costs of the care you may need, be it in your home or at a facility. Pathstones by Phoebe’s continuing care at home program helps you:
• Remain in your home
• Avoid moving to a nursing home
• Preserve your assets
• Unburden family and friends
• Provides a lifetime of benefits
Pathstones by Phoebe is licensed by the Pennsylvania Department of Insurance.
