For your participation in the study (up to four study visits over 12 to 26 weeks), you may receive up to $325 in gift cards. Your participation is voluntary, confidential, and will not affect your care at Southcentral Foundation in any way.

- Manage stress levels with tools from a behavioral health consultant or by attending a learning circle.
- Set up an appointment with a registered dietitian by calling your case management support to assist in planning a balanced, healthy diet.
- Increase physical activity by attending exercise classes with Rehabilitation Therapies and Exercise.
- Schedule a wellness exam with your provider annually.
