Go to the surgeon that does only the specific specialty surgery that you want done. Don’t go to a generalist. Make sure they do at least 50 cases a year of Labiaplasty and just as many Vaginoplasties. Or just as many pelvic reconstructions. Always go to the specific specialist.

Avoid this surgeon: the one that tells you they learned labial surgery in their residency or fellowship. There are no academic training program in the US that actually teach this competently, and these are skills that are learned outside of residencies and fellowships and only obtained from specialty training programs around the world. Don’t be bamboozled and tricked.

Another helpful hint: if they have no Before, and After pictures to show you, then they probably do not do enough cases. They’ll have excuses like patient confidentiality. , or lack of staff or other lame excuse. Buyer beware.

What is Vulvodynia? It is chronic pain and irritation of a woman’s ge***al areas often without a known cause. It hurts, it burns, it’s very irritating, and it makes intimacy almost impossible. Steroids, estrogens, testosterone, and dozens of creams have been used to help relieve symptoms. Botox and PRP and Hyaluronic Acid and Exosomes have been tried with variable results. When all have failed, patients see me for vulvar and vestibular surgery for relief of symptoms. I can help many but not all. Slings for stress urinary incontinence, when it’s affecting your lifestyle and reducing your quality of life, are the Gold Standard treatment performed by Gynecologist, Urogynecologists, and Urologists. They work so very well with about a 90% success rate and with a very low complication rate. These are not the meshes that have caused problems that you may have heard about. These are a thin strip of nylon that’s implanted safely beneath the urethra to support it and become a backboard that helps close the urinary tube when pressure comes pushing from a cough or sneeze. They are minimally invasive and have very low erosion rates. They have been around since the late 1990s and I helped design the Caldera Desara Sling System successfully used in the United States and abroad.

If you leak urine when you cough, laugh, sneeze, jump, and Kegels and pelvic floor Physical Therapy have not helped, or if a prior surgery has failed, come see me. I have placed a thousand of these over 30 years and may be able to help.

Yes, I do augmented repairs! What the heck is it? When childbirth and aging and gravity weaken va**nal tissues, stretching inevitably happens and the bladder and the re**um and the uterus and the top of the va**na start falling down and sometimes falling out. Your pelvis feels heavy, you get a lot of pressure, and a bulge starts protruding from the va**nal opening.

Standard repair using your already weakened tissues and just suturing back together is what most surgeons do. That results in failure of bladder repairs in about 20 to 30% or even higher. I just don’t think suturing stretchy and weak tissues back together is a great idea. So I use highly screened and safe cadaver tissues for my “augmented repairs“ as REINFORCEMENTS. This lowers your failure rates to about 5 to 10%. Not perfect but so much better. I have 30 years of experience with exceptionally low complication rates and no rejections at all in my patience.

This is the reconstruction and functional repair that I do that often precedes the Cosmetic and Aesthetic work that follows.

Yes! I love Pelvic Reconstructive Surgery! When the bladder and the re**um and the uterus or the top of the va**na is hanging out like an inside out sock I get very excited because I love these complex repairs. I love incontinence and leaky bladder procedures and have invented many of the devices to take care of these problems. I always take care of these issues first before any cosmetic labiaplasty or Vaginoplasty. I put them all together and call it my Unified Approach.
