As our Patient Care Coordinator, you will:
✔️Schedule home visits, intakes, assessments, and caregiver shifts
✔️Maintain up-to-date client records and care plans
✔️Serve as a point of contact for clients, families, and caregivers
✔️Answer phone calls, emails, and client/caregiver requests in a professional manner
✔️Follow up with clients to ensure satisfaction and care continuity
✔️Support the care team with documentation, compliance, and client updates
