Vitamin B12 deficiency
Vitamin B12 (Cyanocobalamine) is important for maturation of red and white blood cells (RBC's, WBC's) within the bone marrow. Vitamin B12 is also crucial for normal function of the peripheral nervous system. When low (called Pernicious Anemia)
patients can present with lack of energy, low motivation. Blood work will show normal RBC count BUT the MCHC will be higher than normal. Prolonged deficiency of B12 can cause a "Peripheral Neuropathy" manifest by "Pins and needles" especially in the feet. Vitamin B12 deficiency is most commonly seen in senior citizens and strict vegetarians. Vitamin B12 is stored in liver and levels can never be high enough to cause toxicity.
Interestingly, we are now seeing Vitamin B12 deficiency presenting in younger middle aged patients who are non-vegetarians. These patients are frequently asymptomatic, and if not for a high MCHC are fine. Along with the increase of Vitamin B12 deficiency acid blocking medications such as Pepcid, Nexium, Omeprazole are being prescribed more and more frequently for acid reflux. All of these medications are safe and very effective. This is ONLY an assumption, but Vitamin B12 found meat is attached to a carrier protein called "R protein". R Protein combines with the Vitamin B12 within our saliva, stabilizes it and transports it to the stomach, where the stomach acid could dislodge the R-protein where the Vitamin B12 is passed on to another transporter Intrinsic Factor (IF) which transports the Vitamin B12 into the small into the small intestine where it is absorbed and makes its way into the bone marrow and peripheral nervous system.
One possible explanation (this is my assumption only) is when the gastric acid is lowered the Vitamin B12 attached to the R-Protein may NOT be detached. If this is true (again, my assumption only) then Intrinsic Factor (IF) will NOT be able to bind to the Vitamin B12. Pernicious Anemia is caused by LOW Intrinsic Factor concentration. Without IF, Vitamin B12 is NOT absorbed and is excreted with next bowel movement.
Regardless if my assumption is correct. The solution is simple, start taking B12 supplements, or better yet Vitamin B12 injections ( This bypasses the gut). Vitamin B12 is safe what your body does not use is stored in the liver. Vitamin B12 toxicity does not occur because what is not used is stored by the liver. Do NOT stop taking your gastric reflux medications
( Nexium, Omeprazole, Pepcid, Tagamet). These are great drugs with great safety record and are safe and effective. Continue to take your reflux medications, because the adverse effects of untreated gastric reflux disease is far worse (Esophageal cancer). CONTINUE TO TAKE YOUR REFLUX MEDICATIONS (Nexium etc.). Vitamin B12 supplements are safe and effective or get a yearly or even every 6 month injection of Vitamin B12 by your Physician.

Herbal fun fact: Where does the "Mad Hatter" in the "Alice and Wonderland" fairy tale come from?
In 19th century London, men as today wanted to look prosperous. The hats worn by men conveyed status and "breeding".
A hat could definitely "make the man" back in those days.
Hat makers were quite sought after. As part of making a hat, Mercury was used within the lining. Now, I do not know why Mercury was used as a hat liner but it was common. The amount and concentration of Mercury was very, very small.
So for the most part men never suffered from mercury poisoning. Like clothing designers of today, the best way to market your product was to get it out in public (No, TV, radio, or internet). The best way was to wear your hats out in public to help drum up business. Therefore, hat makers were on a daily basis of years then decades of being exposed to Mercury. Although in very, very, very amounts and concentration, the hat maker's hands , the forehead and scalp received a LOT of mercury exposure.
Mercury does have medicinal properties (To be discussed in future posts) but only in very, very, very small amounts eg.
.00001). However, when used in higher concentrations, Mercury poisoning is characterized by "Dementia". Mercury will collect within the Frontal Lobes of the brain. The Frontal Lobes of the human brain controls our personality, affect, decision making.
In the 19th century, "Dementia" was referred to as "Madness".
Thus, hat makers who were on a daily basis for years and decades exposed to Mercury. And when they wore the hats they made, they would sweat. Well, their perspiration INCREASED the absorption of the Mercury within the lining through the skin and brain. Over decades the Mercury builds up within the Frontal Lobes of the hatters resulting in Dementia, which they called "Madness". Hence, the "Mad Hatter" character in the story "Alice and Wonderland"...
