Home health may be necessary for your recovery following a stay at a hospital or rehabilitation center, post-op care, if you have been diagnosed with a new illness or disease, significant change in your medical condition, status, or medications, if you have fallen recently or at risk of falls, if you have a chronic disease requiring clinical management (CHF, COPD, MS, Diabetes, or Alzheimer's), IV therapy, or wound care. Bridgeway provides skilled nursing, physical therapy, speech therapy, occupational therapy, home health aide, and medical social work.
