Did you know?

Small Bowel Obstruction ( ) can be diagnosed with with an excellent sensitivity (92%) and specificity (92%) relative to CT scan and surgery.

How?

Lawn mower over the abdomen (or expert tip, start where the pain is) and look for:
Diagnostic sign:
-3 adjacent dilated small bowel loops (>2.5 cm) with adjacent collapsed bowel
Adjunctive signs:
-"to and from" peristalsis (the bowel contents go back and forth as they hit the obstruction), Fluid adjacent to the bowel (tanga sign), Fluid filled bowel with visible plica circularis (keyboard sign)

Cool!

But... why would I do it? Don't the surgeons need a CT scan anyway?

True, but let me tell you about my last patient with SBO.

She had abdominal pain and vomiting. Her bowel movements weren't great, but she had some diarrhea the day before. She thought it was probably a gastroenteritis but she had a funny feeling inside it was something more. 30 years earlier she had a bowel obstruction and something about it reminded her of that time. She was almost embarrassed at being in the ED and was concerned about "wasting my time". Her exam had some RLQ tenderness but overall wasn't terrible.

So I pulled out the and got the image below, showing SBO. The patient was "thrilled" in that her visit was vindicated. She felt better already knowing her diagnosis. The relief on her face was palpable. The CT would ultimately take hours to be done.

So what could we do in the meanwhile?
-Get her from a hallway bed into a room
-Starting NG tube suction to provide relief
-In some systems we could get her admitted to the hospital faster.

But most of all it provided both the patient and myself with diagnostic closure. We both knew what was going on. She could feel mentally better and physically better faster, and I know her diagnosis and have a treatment plan prepped. My limited brain power could now move onto thinking about other patients.

Sometimes it's those less tangible benefits of POCUS that make the most difference.
