It’s no surprise that scleral lenses are a big part of my work, and something I’m genuinely passionate about.

Over the years, I’ve had hundreds of conversations with patients (and colleagues) who are curious, skeptical, or just overwhelmed by the idea of specialty lenses. So I wanted to share the questions I get and why these lenses are such a game-changer in my practice.

▫️ 1. What are scleral lenses, exactly?

They’re large gas-permeable lenses that rest on the white part of your eye (the sclera) and create a fluid reservoir over the cornea ideal for comfort and clarity in complex cases.

▫️ 2. Who are they for?

Mostly patients with keratoconus, severe dry eye, post-surgical corneas, or irregular astigmatism. But honestly, anyone struggling with vision or comfort issues that regular lenses can’t solve might be a candidate.

▫️ 3. Are they actually comfortable?

Yes, surprisingly so. Most patients are nervous at first, but after a few days of adapting, they tell me they forget they’re even wearing them.

▫️ 4. How do you put them in?

With preservative-free saline, a steady hand, and a little practice. I make sure each patient feels confident before they leave the office.

▫️ 5. And what about the cost?

They’re custom-designed, so yes, they’re an investment. But there are ways to make them more accessible, including insurance, HSAs, and payment options. Most patients find the benefits absolutely worth it.

Scleral lenses aren’t for everyone but when they’re the right fit, they can truly change lives.
If you’re an eyecare practitioner curious about adding them to your toolbox, or a patient wondering if they’re right for you, I’m always happy to talk.

This is a real example of what targeted treatment can do.
The top photo shows the eye before treatment with XDEMVY — note the inflammation and collarettes along the lash line.

The bottom photo was taken after a full course of XDEMVY. Clearer lid margins, less redness, and a noticeable improvement in patient comfort.

XDEMVY is the first FDA-approved treatment specifically for Demodex blepharitis.

Always grateful to be able to offer cutting edge treatments to my patients.

Prosthetic vs. Cosmetic Contact Lenses: What’s the Difference? 🤔

While both prosthetic and cosmetic contact lenses alter the appearance of the eyes, they serve different purposes.

✨ Prosthetic Contact Lenses
• Designed for medical or therapeutic use.
• Help individuals with eye conditions, injuries, or irregular iris or cornea appearances.
• Can improve vision or reduce sensitivity to light.
• Require a prescription and custom fitting by an eye care professional.

✨ Cosmetic Contact Lenses
• Primarily used to enhance or change eye color.
• Do not provide medical benefits but can have prescription options for vision correction.
• Available in enhancement, opaque, or special-effect styles.
• Should always be fitted by a professional to ensure eye health and safety.

Even if you’re considering cosmetic lenses for aesthetic purposes, consulting an eye care specialist is essential to avoid complications and ensure proper fit.

Our Story

Dr. Elise Kramer is a residency-trained optometrist in Miami FL who specializes in ocular surface disease and regular and specialty contact lens fitting.

Her Doctorate degree was awarded in Optometry from the Université de Montréal in 2012 where she received a grant from the Scholarship Program of the Québec Ministry of Education for short-term university studies outside of Québec. Receipt of this scholarship award is based on academic performance. In the summer of her second year, she participated in a humanitarian mission in Laayoune, Morocco where she helped hundreds of impoverished people by giving them free eye examinations and glasses corresponding as closely as possible to their prescription.
