It’s probably not a clog that’s causing the problem. It’s most likely breast tissue inflammation that is causing pressure on the milk ducts and can also cause milk stasis.
If you have engorgement or hard spots, it can cause more damage to treat the breast as though there is a clog to squeeze out.

And sunflower lecithin isn’t the best choice either. It can cause GI issues in both mom and baby when there are better alternatives that don’t (like supplementing choline as needed and breast health probiotics).

If you’re seeing clumpy or stringy milk, it can be a first sign of infection that needs to be addressed.

Fever or flu like symptoms when you’re also having these breast issues? Message your provider.

Lumps, engorgement, breasts not feeling well drained, frequent stringy milk, frequent mastitis - get to an IBCLC, don’t wait it out.

Is it a supply issue or a transfer issue? Let’s find out.

Is it a supply issue or a transfer issue?
Let’s find out.

If your baby isn’t growing well on breast milk alone, it’s not because your milk is missing nutrients. Breast milk is designed to be complete nutrition for babies. It’s the gold standard. 💛

When we see slower weight gain, the issue is usually volume, not quality. Babies may not be getting enough ounces because of things like:
⏳ Limiting time at the breast: cutting feeds short can mean baby doesn’t get enough. 15 minutes a side can limit some from getting a full feeding
📅 Strict schedules: bodies don’t run on timers, and babies especially need flexibility. Babies want to eat every 1, 2, or 3 hours and not in perfect increments. They cluster feed more often during growth spurts as well
🔄 Eat/play/sleep routines: these can unintentionally space feeds too far apart. Most babies eat a breastfeeding meal when they wake up, play and socialize, and breastfeed a snack before going down to nap
🌙Sleep training or skipping night feeds: some babies still need those calories overnight. Night feedings account for 20% or more of breastfeeding baby’s total calorie intake through the first birthday! Over 80% or babies wake to feed!!

👉 The takeaway: breast milk doesn’t come in “skinny” or “fatty.” If baby needs more growth, we look at how often and how much milk they’re actually getting, not whether your milk is “good enough.”

Your milk is enough. đź’Ş Sometimes babies just need more access to it.

What have y’all been seeing when you’re scrolling?
I’ve been seeing some things! So I want to inject a little balance.
1. Every feeding does not have to be a full feed. While feedings eventually stretch out, don’t pressure yourself (or baby) to have long stretches without eating as the norm.
2. Maybe I’m biased because I don’t like math. But ignore all fl**ge sizing instructions that include “and then add …”
3. If you’re struggling or wondering or questioning, reach out to a skilled IBCLC. You don’t have to have a doctor’s referral or permission. You can just been seen and work on issues earlier, and get relief and peace of mind earlier, too.

If they say you have to pump and dump for … insert medication here … always get a second opinion. In the majority of cases you do not need to. There are, of course, meds that are not recommended to use while breastfeeding.

An antibiotic? Many are compatible with breastfeeding, or it could be adjusted to find one that is.
Pain medicine? Many are compatible with breastfeeding.
CT scan with contrast? For typical scans, the molecules of the contrast are too big to pass into your milk at all.
Anesthesia, whether general or moderate sedation does not mean you have to dump your milk! Those medications are meant to only last for the duration of the procedure and they do not somehow secretly get stored in your milk.
Watch here for more specific info on this topic. In the meantime, if your provider has not sourced the information from specialized resources such as Infant Risk or Hale’s Medications and Mother’s milk or e-lactancia, there could be missing information.
Many providers feel it’s just safer to err on the side of dumping milk. However, when your baby is doing well latching to the breast or drinking your expressed milk we should be careful to interrupt that only when necessary.
