What exactly is diaphragmatic breathing and why is it so important for proper lymphatic functioning?

The diaphragm separates the lungs from the abdominal cavity and in our "normal" breathing it slightly moves down to create space for the expanding lungs as we inhale and retracts back up and the lungs empty.

In deep diaphragmatic breathing, the movement of the diaphragm is extended further into the abdominal cavity which not only is highly efficient but also provides a host of benefits such as:

1) Massaging abdominal organs and tones muscle of the abdominal wall
2) Improves digestive, metabolic and excretory functions
3) Reduced strain on the heart
4) Aids in lymphatic transport and drainage via stimulation of the thoracic duct

As we prepare for breast cancer awareness month coming up in October, we want you to know how to recognize the signs and symptoms of lymphedema and all treatment options that are available to you.

Lymphedema is a chronic condition that can some times occur following treatment interventions for some cancers. A common question we hear from our new lymphedema clients is, "what can I expect during the treatment process for lymphedema?"

Here we will cover the "gold standard" for lymphedema care known as Complete Decongestive Therapy and what you can expect in the first few months of care.

Complete Decongestive Therapy is composed of two main phases.

The first phase is a decongestive phase, where the excess lymphatic fluid is removed through a combination of short stretch bandaging, manual lymphatic drainage, diaphragmatic breathing, proper skin care, patient education on precautions, exercise and appropriate sleep. This phase can last anywhere from 2-8 weeks – dependent on patient preference and availability.

The second phase is a maintenance phase, where the patient is fit for the appropriate compression garment as determined by his/her CLT (i.e. sleeve and glove, sleeve and gauntlet, compression thigh highs, knee highs, panty hose, abdominal compression top, facial compression-wear, breast compression bra) in conjunction with proper skin care, practicing lymphedema precautions, diaphragmatic breathing, exercise and continued appropriate amount of sleep/night. This phase is life long.

Compression is an incredible tool in the treatment of lymphedema BUT there are so many other tools we can use to support healthy flow of lymph in the presence of lymphedema (swelling that occurs when lymph fluid builds up in between the cells of our bodies) in addition to compression.

One that we pride ourselves on at The Training Station PT is movement! We recommend avoiding high intensity exercise in the heat and starting easy. Build up intensity slowly, no > than 10% increase in distance/weight/reps per week and remember you can always ask us for tips on suggested movement or exercises that are safe and effective for your body.

Fun Fact! Unlike our other systems that have a natural flow or rhythm (like the cardiovascular system) our lymphatic system relies on gravity, breathing and muscle contraction and movement for proper flow of lymph.

This is one of the reasons we love using suspension tools like aerial slings and Red Cord which not only supports gentle accessible movement but also allow us to use gravity to our advantage!

Did you know there is a stage 0 or a latency stage of lymphedema?
In this pre-syndrome stage of lymphedema, lymph is not properly flowing and there are not yet any visible signs of edema (swelling) but people in this stage may notice some of the following symptoms:
1) General feeling of fullness, heaviness, or tightness of the skin in the affected limb or area of the body
2) Aching or new strange sensations
3) Mild tingling or "pins and needles" sensations
These symptoms may be present for a few months up to several years before moving into the first stage of lymphedema. The good news is, at this stage we may be able to intervene with dietary changes, movement, PT, medication and compression to reverse the progression.

Lymphedema is a congestive condition that occurs when there is a blockage in the lymphatic system (a necessary component to our immune system) which helps drain excessive fluid from our bodily tissues. These blockages occur when the lymphatic system becomes damaged which can happen from a wide range of factors such as:

1) Physical trauma like deep cuts or bruises
2) Surgeries or radiation treatments
3) Infections
4) Vascular conditions
5) Other factors such as lack of movement, allergic reactions or genetics
