Advance directives are self-care. Full stop.

Most people put them off because it feels heavy. But sitting with your values, deciding who speaks for you, and putting it in writing? That's one of the most loving things you can do for yourself and for everyone who cares about you.

Here's what most people don't realize: a properly crafted living will goes far beyond CPR. It guides your healthcare team on antibiotics, dialysis, artificial nutrition, artificial hydration, pain management, sedation levels, and ICD use. Optional directives for dementia, mental health, voluntarily stopping eating and drinking, and others exist, too, and most families never hear about them.

Peter at Compassion Crossing, a holistic registered nurse, spends the first 90 minutes of your initial meeting working through a 16-page values worksheet, 10 decision aids, and the legal forms with you, and then stays available for on-site meetings and calls for 12 full months to ensure every document truly reflects you.

You don't have to figure this out alone.

How Death Doulas Are Bridging Critical Gaps in Hospital End-of-Life Care

Death doulas are emerging as powerful partners in hospital-based end-of-life care, providing non-medical holistic support that complements clinical teams. These trained companions fill critical gaps in emotional, spiritual, and social care that healthcare professionals often lack time or training to address.

Most people think a living will is just about CPR. It's not even close.

A properly crafted living will guides your healthcare team on antibiotics, dialysis, artificial nutrition, artificial hydration, pain management, sedation levels, and even blood sugar management. There are also optional directives for dementia, mental health, voluntarily stopping eating and drinking, and other situations most families never hear about.

Many assume an attorney is all they need. Attorneys absolutely produce legally sound documents, but most don't have a healthcare background, and that gap matters. Optional directives for dementia, mental health, and voluntarily stopping eating and drinking rarely make it into attorney-prepared plans simply because most attorneys have never heard of them. Value-based medical decisions, the kind that shape what care actually looks like in your final chapter, require a depth of healthcare understanding that falls well outside the typical legal skill set.

This video explores the ethical issues surrounding Medical Aid in Dying (MAiD), including how laws might mislead the public and families by changing death certificates to list a different disease as the cause of death. It also discusses the problem of low healthcare literacy, where many people do not understand that being labeled terminal does not mean imminent death; a terminal prognosis is only a clinical estimate without certainty. Dying is often not painful, and palliative care can manage symptoms for years until hospice care becomes necessary. Between these two types of care, patients can usually stay comfortable and enjoy a good quality of life until natural death.

When AI Gets Hospice Documentation Wrong: Why Clinical Expertise Still Matters

Artificial intelligence is transforming hospice clinical documentation, but these tools often hallucinate, guess at answers, or provide outputs designed to please users rather than meet CMS compliance standards. Hospice nurses and clinical managers must understand proper eligibility-driven documentation to prevent audits and ensure deserving patients receive care.

Here's where it gets complicated: Kentucky's MOST form looks straightforward, but sections A through D function like radio buttons, one selection only, and many people treat them as checkboxes. Section B, especially, is a landmine. A single wrong selection there can quietly override values the living will took great care to establish, and incorrectly completed forms slip through the system more often than most people realize.
