Women often ask me about hybrid reconstruction and when this might be a useful option. Hybrid reconstruction means we combine natural or autologous tissue with an implant to create the final reconstructive result. Using this option, the inner surface of the breast is restored with the patient's natural tissue, and the remainder of the breast volume and projection is achieved from a breast implant buried below the natural tissue, much like a breast augmentation.

Who may be a good candidate for hybrid reconstruction?

→ Women who have smaller donor tissues available but desire the natural result of autologous tissue.
→ Women who have experienced contour deformities with implant-based reconstruction.
→ Patients who have had significant implant visibility or other irregularities from implant-only reconstruction.
→ Women who desire smaller donor site scars or less visible donor sites.
→ Breast cancer survivors who have had radiation and will benefit from natural tissue but still desire additional volume from an implant.

I wanted to continue my discussions of the radiated breast! It's so important to understand the potential impact of radiation on your choice of reconstructive method. When autologous or natural tissue is utilized upfront as the primary method of reconstruction, progressive problems associated with implant-based reconstruction after radiation can often be avoided. In my last post, I shared images of progressive capsular contracture, skin and soft tissue fibrosis, and breast asymmetry resulting from years of radiated tissues responding to a foreign breast implant.

In this case, the patient underwent autologous tissue reconstruction with a bipedicled DIEP flap, using most of the lower abdomen to reconstruct the left breast. This was staged for a few months with an intermediate tissue expander to hold the skin envelope while she underwent radiation and to avoid radiation to the natural tissue. The expander was exchanged for natural tissue 3-6 months after the completion of radiation.

Her DIEP flap will work for her for the rest of her life! Over time, the breast will only look and feel more natural as non-radiated natural tissues soften and restore what was damaged by mastectomy and radiation. We are both thrilled about her transformation.

Many women present to me looking for revision surgeries to exchange their breast implants as they may become stiff, hard, and contracted over the years after whole breast radiation. Radiation is often part of the treatment regimen for breast cancer and an important part of reducing recurrence risk. If an implant has become painful and contracted, you still have options! The best option generally isn't a new breast implant since it is likely the same problem will occur again.

Removing the implant and bringing natural tissue with a new blood supply to the radiated chest will often help improve the quality of radiated tissues and scar contractures by restoring vascularity and lymphatic ingrowth. With natural tissue, the breast tissue quality improves over the course of your lifetime, as new blood supply works its way into damaged tissues.

My favorite follow-ups are patients who return years after exchanging their radiated implants for natural tissue. In these cases, the tissue transfer continues to work for them, becoming softer and more natural feeling over time.

The options for breast reconstruction can be confusing. It is most important to identify a board-certified plastic surgeon specializing in reconstructive surgery and a surgical oncologist specializing in breast to guide you based on your specific medical need, treatment plan, and reconstructive goals. Once you've identified your team, evaluate each reconstructive option available. Options for breast reconstruction range from implant-based to natural (autologous) tissue.

→ Implant-based reconstruction uses some form of breast implant to re-create the appearance of the breast. This is sometimes performed in stages for the best final result.

→ Autologous reconstruction utilizes your own tissue to reconstruct the breast. Tissues are transferred from one part of the body and shaped to re-create a natural appearing breast. Tissues are often taken from an area of excess, including the lower abdomen, inner thigh, upper buttocks, or love handle area. It's important to be mindful of where the tissues are taken from, making sure that region can be closed in an aesthetically beautiful way, as for a tummy tuck, thigh lift, or buttock lift.

→ Hybrid reconstruction combines these options, utilizing one's own tissue for a natural result and combining it with an implant for added volume or projection.

Remember, aesthetic flat closure and no reconstruction is an option and a choice as well! A plastic surgeon is often involved to achieve the best contour and appearance.

Today, I am sharing one of my favorite cases demonstrating the remarkable power of autologous tissue reconstruction. Under the care of a different team, this patient originally underwent a DIEP flap to reconstruct her right breast and a breast implant to reconstruct her left breast. She was unhappy with the asymmetries of the breast reconstructions, shape and contour of the breasts, and the overall shape of the abdomen.

Here, I used a small lumbar flap to add volume to her right breast lower pole to improve the contour and lower pole volume. I converted the left breast implant to a lumbar flap entirely. Additonally, I performed some abdominal contouring revisions to improve the shape and contour of the abdomen.

I love this case because it really demonstrates the importance of choosing the right flap based on body habitus and aesthetic goals. Additionally, symmetry is often better achieved by using the same form of reconstruction for both breasts, which I tried to ultimately achieve here.

She loves her results, and I'm thrilled to make this happen for her!

Patients are often SOO nervous that a natural tissue reconstruction means lots of scars and an unnatural "Frankenstein" or "patchwork" appearance. This couldn't be further from the truth! My patient here is just one week out from her DIEP flap revision surgery and is looking lovely already. She has some natural swelling and bruising from surgery that will resolve in a few weeks. We are so grateful she allowed us to share this early result!

What happened here? This patient underwent a double mastectomy for breast cancer with immediate tissue expander insertion. This allowed me to shape the breast and for the skin to heal while preserving the ni**le. She then went on to DIEP flap reconstruction, followed by minor revisions for contouring, fat grafting, and scar revisions. Her reconstructed breast will age and change as she does over time, leading to a natural result over the course of her lifetime.
