Not all sleep training involves Crying It Out (CIO).

While CIO is safe, effective, and works quickly for most babies when done correctly, it is not for everyone.

CIO can be really hard for parents, and you have to be prepared to tolerate some tears (theirs and possibly some of yours)!

I did a behavioral approach with my LO, and she started sleeping through the night right at 7 months, the day before I was at my wits end and about to start CIO sleep training.

✨504 vs. IEP 🤔✨

Many kids need extra support at school to access their appropriate education. IEPs and 504 plans are designed to do just that (in the public schools) and are available as early as 3 years+ through age 21.

Deciding which plan to pursue can be a tricky question for some families. This is especially true for kids with ADHD who often have co-occurring challenges such as learning disabilities or anxiety.

I’ve teamed up with THE CHILDHOOD COLLECTIVE today to walk you through some of the similarities and differences between the two.

Big (simplified) picture:

🔘Think of 504s as broader accommodations to access the regular curriculum (eg., extended time, preferential seating, use of a scribe) due to (mainly) medical condition(s) that impact learning. You generally need a diagnosis from a doctor to access a 504 plan. Usually, 504 services only apply during the regular school year.

🔘Think of IEPs (Individualized Education Programs) as just that - an individualized way of modifying access to and what the curriculum looks like. It has individualized goals & objectives that require monitoring throughout the school year (and beyond for some). A diagnosis isn’t always necessary for an IEP, as the school categorizes students based upon their own disability categories. IEPs tend to offer more legal protections, accountability, and parent involvement, as well as being more individually tailored for the student. While most IEPs start in Kindergarten, some kids will qualify as early as age 3!

Of course, there’s more that goes into the 504 vs IEP decision than can fit in these little circles - it depends on what the student’s needs are and how to best meet those needs.

✨All About Selective Mutism✨

I teamed up with the amazing Speech and Language Pathologist Mrs Speechie P on this topic as there’s A LOT of confusion about Selective Mutism out there!

First things first: Selective Mutism is NOT a choice - children are not willfully staying silent, it’s literally a brain-based (amygdala) process whereby your nervous system gets so overloaded with anxiety that you PHYSICALLY cannot speak.

It’s the quintessential freeze of the “fight, flight or freeze” response to perceived dangers.

It’s most commonly diagnosed between 3 to 8 years of age, and is an anxiety disorder that is often linked specifically to social anxiety.

It’s really important to get treatment as it doesn’t usually go away on its own!

Was a pleasure teaming up with Mrs Speechie P on this!

I teamed up with Mental Health Matters with Dr. Jenicka to talk about the differences in a speech and language evaluation and a neuropsychological evaluation.

A lot of people don’t know there’s some overlap- but there is! A key difference is that Neuropsychs are really trying to break down the whole brain-based areas of strength and weakness across domains of functioning, which overlaps to some degree with SLP evals because language is a part of this. And then SLP Evals are much more nuanced and focused on the different areas of strength and weakness within the speech and language domains. A deeper dive into communication and language.

I was featured for comment in Well+Good about how some people experience emotional numbness and an inability to cry as common (but lesser known) symptoms of depression and stress (or PTSD and BPD). It's an interesting read given the collective stress we have all undergone this past year!

Just finished up my Instagram series on things to consider when seeking mental health treatment and services.

It’s important to decide between going the therapy Vs. Medication versus combined approach AND also your insurance benefits and time etc.

Research shows for many people with mood and anxiety disorders that therapy alone or medication alone are about equally effective as each other while the combination of therapy and medication has the greatest reduction in symptoms.

There’s still a lot we don’t know about the brain. However, many mental health conditions are thought to result from an imbalance of neurotransmitters (chemical messengers) in the brain. Psychotropic medications are thought to address those imbalances - but it can be as much an art as it is a science and sometimes you might need to try more than one to get relief.

One of the easiest ways to access basic psych meds (think everything aside from antipsychotics and mood stabilizers) is through your P*P. Primary Care is responsible for most of the antidepressant prescribing in the US, although they are not specialists and will have varying levels of comfort and expertise with psych prescribing.

If you’re hitting roadblocks it’s best to seek specialty psychiatric care (be sure to check if you need a referral first if using insurance) which means usually a psychiatrist or psychiatric nurse practitioner (in a few states trained psychologists can also prescribe).

Another option for those facing long wait lists for specialists or wanting to try something different would be clinical trials. You can help expand scientific knowledge and others while potentially helping yourself. Not all clinical trials are for experimental drugs, many can be for already FDA approved medications.

Happy New Year! Or is it?!

There is a TON of pressure around New Years to set and declare a resolution to become a “better” you. There’s a multibillion diet and exercise industry depending on people’s (often short lived) resolutions.

If you want to eat healthier and exercise more, cut alcohol or carbs or do one good deed a day that’s great - just remember you’re fab the way you are, and we are still in pandemic mode. So cut yourself some slack and maybe make the resolution to love yourself as you are this year.

2020 is finally over! 🥂🍾🎉
