🩺Rethinking the the Glasgow Coma Scale. Meet CBI-M.

After 50 years of using the Glasgow Coma Scale to assess traumatic brain injuries (TBIs), a new framework may finally bring actionable nuance to TBI evaluation. The CBI-M system integrates four diagnostic pillars: clinical, biomarker, imaging, and modifiers.

This could mean no more vague “mild,” “moderate,” or “severe” labels. Instead, clinicians would assess eye, verbal, and motor responses, use blood biomarkers to detect cellular damage, and leverage imaging to guide care. The framework also considers patient-specific factors like prior TBIs, medications, and mental health.
Why does this matter? The new system aligns diagnosis with the complexity of TBI recovery, ensuring patients get the care they need—whether in acute settings or long-term follow-up.

A sweeping meta-analysis of 57 studies involving 400,000+ adults found that older adults who use digital technology had 58% lower odds of cognitive impairment and a 26% slower rate of decline compared with non-users. The findings dismantle the “digital dementia” narrative and suggest that tech use may build a “technological reserve” that supports cognition.

How? Researchers point to three key mechanisms: complexity (engaging in challenging tasks), connection (fostering social ties), and compensatory support (like GPS for navigation). But there’s a catch—mindless scrolling won’t cut it.
