🏥 Today we celebrate health centers’ commitment to understanding and meeting the unique needs of their communities—providing accessible and quality care.
In California, the Population Health Management Initiative (PHMI) exemplifies this commitment. A collaboration between the Department of Health Care Services, Kaiser Permanente, and Community Health Centers (CHCs), PHMI supports a cohort of 32 CHCs, which serve 1.1 million people, to improve their population health management capabilities and align with Medi-Cal policy and payment reform initiatives.
PHMI’s goals are to improve quality and fairness in health outcomes, enhance access through team-based care, strengthen patient engagement and experience, and advance technology to support population health at CHCs.
Known for its training and healthcare transformation expertise, was contracted by Kaiser Permanente to lead the Practice Transformation Partnership, which is part of the PHMI, a collaboration with the Institute for Healthcare Improvement (IHI), the Center for Care Innovations (CCI), AllianceChicago, and the Center for Excellence in Primary Care (CEPC).
Through coaching, subject matter expertise, peer-learning opportunities, and health information technology (HIT) support, the cohort significantly improved capabilities in seven of the eight domains measured in a structured self-assessment, including care team and workforce, empanelment and access, technology and data infrastructure, patient-centered care, the business case, leadership and culture, and social health.
