Piriformis Syndrome: Long vs. Short – Quick Rehab Tips!

If you're dealing with piriformis pain, knowing whether it's long or short piriformis syndrome can help with the right treatment!
🔹 Short Piriformis Syndrome (Tight & Overactive)
👉 Symptoms: Deep buttock pain, tightness, possibly radiating to the leg

✅ Stretch It Out:
Seated or lying piriformis stretch (pull knee toward opposite shoulder)
Figure-4 stretch (cross ankle over knee, pull thigh toward chest)
✅ Massage & Release: Foam rolling or trigger point therapy can help.
🔹 Long Piriformis Syndrome (Weak & Overstretched)
👉 Symptoms: Deep glute discomfort, instability, aggravated by prolonged sitting.
✅ Strengthen It:
Glute bridges & clamshells to activate surrounding muscles
Hip abduction exercises to restore balance

✅ Avoid Aggressive Stretching: Overstretching can worsen symptoms!

✅ Rest & Activity Modification – Avoid repetitive gripping, texting, or lifting with your thumb.
✅ Splinting – A thumb spica splint can help immobilize and reduce strain.
✅ Ice Therapy – Apply ice for 15–20 minutes to reduce pain and swelling.
✅ Gentle Stretching – Try the Finkelstein stretch:
👉 Make a fist with your thumb inside your fingers.
👉 Slowly bend your wrist downward.
👉 Hold for 15–30 seconds, repeat 2–3 times.

For SI joint pain, there are two main treatment focus:
1. Re-align the Joint to decrease pain and inflammation
2. Strenghthen/stabilize to prevent injury and get back to activities of daily living

There are 4 main exercises lying flat:
Note: HOLD 5 SECONDS, REPEAT 3 TIMES

1. PUSH/PULL (LEFT LEG ON TOP):
Place a dowel or broomstick under one knee and above the other and hold it with both arms while you are lying down on your back. Press your legs into the stick while using your arms to prevent the dowel from twisting.

PUSH/PULL (RIGHT LEG ON TOP):
Place a dowel or broomstick under one knee and above the other and hold it with both arms while you are lying down on your back. Press your legs into the stick while using your arms to prevent the dowel from twisting.

2. BALL SQUEEZE: 5sec hold

3. BELT PULL-APART: Lying on your back with both knees bent wrap belt around knees. Open both knees and hold 5 sec.

Common Causes:

Injuries: Broken bones, sprains, or strains.
Nerve Issues: Pain with tingling or numbness (e.g., sciatica).
Organ Problems: Sharp belly pain (appendicitis, kidney stones)
Joint Pain: Sudden swelling or redness (gout, arthritis).
Chest Pain: Could be a heart attack if paired with shortness of breath or sweating.

When to See your Physician:
