Sometimes what looks like a “deformity”… is actually tension.
And when that tension builds up, it can turn into a true deformity.

Young children with neurological conditions often find clever ways to move when they struggle with increased muscle tone and neuromuscular weakness. Over time, these “work-arounds”, aka compensations, pull unevenly on muscles and fascia, creating tightness and changes in alignment.

After just 5 days in a gait intensive with me, this child’s foot changed dramatically. This was not because of casting or bracing, but because we addressed the root cause.

I like to say: I peel back layers. What’s really restriction, and what’s just tension?

Just ONE week apart.

These comparison photos show how quickly a baby’s head shape can change with early, targeted intervention.

Let’s take a closer look:

✔️ Improved rounding of the left posterior skull — the flat spot is filling in, leading to a more symmetrical head shape. Right-sided fullness seen in the first photo is less pronounced, reflecting better cranial alignment.
✔️ Emerging egg-shaped contour — this natural shape is becoming more defined as the left side rounds out. Note how the white line in the hair has shifted upward—an external marker of a structural change.
✔️ Forehead and hair whorl improvements — initial left frontal bulging and hair whorl shift (due to asymmetry) are normalizing, showing change across the entire cranial vault.

This kind of progress is possible with early referral for torticollis and a whole-body approach. In this case, we saw measurable change after just two sessions and a customized home program that began on day one.

Early identification matters—and targeted PT can make a major difference in a short amount of time.

Every baby’s body tells me a story.

This little cutie came to me just shy of turning 10 months old.

Parents concerns: always *hated* tummy time, not crawling, not transitioning in and out of sitting. Mother told over and over again that tummy time “would get better” and to “just keep practicing”. I’m glad she didn’t listen.

At initial evaluation, he met the cut-off for gross motor delay based on the Alberta Infant Motor Scales, presented with high degree of postural asymmetry, and layers and layers of myofascial tension (previous tethered oral tissues released). Interestingly, I also identified segmental hypo mobility of his spine into extension, starting at his mid thoracic spine. He had a prominent xhyphoid process at the evaluation, directly across being the hinge point of mobility at his spine, with hypomobility above and below.

Treatment: gentle segmental spine mobilizations to hypomobile segments, TMR Tots releases to address myofascial tension and postural asymmetries, neuromuscular re-education with movement within range of control to normalize thoracic curve, segmental control activities, functional strengthening of hip girdle and trunk, and emphasis on rotational movements of ribcage and pelvis via transitional movements and gross motor skills.

6 follow-up sessions strategically spaced + a robust home exercise program, little man is creeping on hands and knees, pulling to stand, cruising, transitioning between support surfaces, walking with a push toy, and standing alone for several seconds. He’s presenting with age-appropriate gross motor skills and very close to taking his first steps alone.

Take away points:
- If tummy time is hard, there’s a reason
- Myofascial and joint restrictions affect babies and can physically prevent a baby from being comfortable in certain position
- Effective infant treatment often involves orthopedic and developmental interventions
- When performed skillfully, pediatric PT is efficient. In the absence of neuromotor impairment, PT for gross motor delay should not last forever!

Stretching the neck is not the most effective way to treat torticollis. There. I said it!

Torticollis is not an isolated neck problem: The myofascial system connects the entire body, from head to toe (literally). Tension in the muscles and fascia anywhere could exert a pull on the neck muscles. In most of the babies I see for torticollis treatment, I see that the “problem” is often in the trunk. I also see asymmetrical head position in infants with tethered oral tissues (aka tongue and lip ties). If the problem is coming from somewhere else in the body, stretching the neck is essentially a bandaid solution. Hence why many therapists find torticollis “tricky” to treat and are often puzzled by how long it can take to resolve fully when using the conventional approach (“stretch what is tight, strengthen what is weak”).

“Our geneticist says it’s very very rare, about 1000 people internationally have been diagnosed.”

I had the pleasure of meeting Cary and June in October of 2022 when I first evaluated June for early intervention physical therapy services. It took a lot of fighting and advocacy, but we have answers now.
