What’s the GUIDE Program All About?

GUIDE stands for Guiding an Improved Dementia Experience. It’s a new initiative from the Centers for Medicare & Medicaid Services, designed to give families extra support when they need it most. The program is aimed at helping people living with dementia—and the family members who care for them—by offering services like:

Respite care to give caregivers a well-deserved break
Care coordination to help you navigate the healthcare system
Dementia-informed support from professionals trained in memory care

Cornerstone Caregiving is teaming up with PocketRN to take dementia care to the next level.

Through a new partnership, we’re joining the CMS GUIDE Model — a program designed to better support individuals living with dementia and the family members who care for them.

What this means:
- 24/7 virtual nurse access
- In-home care and safety checks
- Respite care for caregivers
- Personalized care plans and coaching

In honor of veterans this May, Cornerstone Caregiving is partnering with organizations through our Salute to Service Fundraising Campaign to raise awareness and funds for veteran mental health. 22 veterans die by su***de every day, and the $22 for 22 Challenge is a way to honor them. I’ve donated $22 and challenged 22 friends to follow suit. Let’s show our veterans they are not forgotten. Your donation will go directly to our local VFW right here in Casper.
