Negative-pressure ventilation has been studied as a hemodynamic tool in pediatric postoperative cardiac patients. In a widely cited JACC report, NPV improved cardiac output in children after tetralogy of Fallot repair, supporting the concept that negative pressure can assist circulation when the heart is struggling. For pediatric cardiac teams, ventilation strategy matters. Because intrathoracic pressure can influence preload and right-heart filling, the choice of ventilatory approach is an important clinical consideration. BCV’s negative-pressure mechanism provides noninvasive ventilatory support while operating through a different physiologic approach than positive-pressure ventilation. In appropriate patients and at the clinician’s discretion, this may be a useful option when hemodynamic considerations are part of the overall care plan.
