This patient came into my office knowing something felt off—but she couldn’t quite pinpoint what it was. She just had a sense that her features weren’t in balance.

That’s something I hear often. Many patients tell me they don’t love how they look in photos or dislike their side profile, even if they can’t explain why.

In this patient, we enhanced her lower face to bring it into better balance with her upper face. I used facial filler in just one visit to get her to what I call “Ground Zero”—a place of harmony. Sometimes it takes two sessions, but once we reach that point, maintenance is typically just once every 1–2 years.

Filler does last longer than that, but it gradually dissolves—and of course, we don’t stop aging. That’s why a subtle, long-term approach works best.

I approach the face as a plastic surgeon—not just someone adding volume, but someone carefully restoring what’s been lost and reshaping for overall harmony. Filler should never make someone look overdone. It should make them look balanced, refreshed, and naturally beautiful.

As I look at this photo, I can’t help but think about all the beautiful patients I’ve had the privilege of caring for over the years.

I’m celebrating 24 years of being Board Certified by the American Board of Plastic Surgery and 19 years in solo private practice at Judith Gurley Plastic Surgery. I’ve been blessed with so many amazing patients who have shown such gratitude and sincere appreciation along the way.

I have to say, it would be difficult to be a plastic surgeon if I didn’t have the gift of good hands and an artistic eye, but it would have been impossible to be a plastic surgeon if I didn’t have the gift of empathy and compassion.

I’m deeply grateful for the opportunities I’ve been given to pursue my goal and live my dream. The combination of art, science, and the study of the human body brings together everything I love.

In this case, the smaller breast was enhanced using the patient’s own body fat (fat grafting), and both breasts were reshaped with a lift to restore balance and symmetry.
