It's a fact the importance of patient and family teaching in healthcare remains urgent. I want to shed light on the useful tool of focused assessments potential of effectiveness when utilized at home. Focused assessments are widely utilized by healthcare providers to facilitate more detailed health data not only for progressive direct care but also to aid in communicating healthcare information within nurse to patient relations. A focused assessment creates a detailed nursing assessment limiting target to specific body system or area of the body related to the presenting problem and chief complaint. Focused assessments generally can be broken down into four steps. 1. Notate all health history of targeted system such as treatments, medications, hospitalizations, as well as environmental exposures. 2. Do a local skin check for any changes in color, temperature, swelling, and pain. 3. Detail all current symptoms and any other perceptive subjective health data. 4. If available include any collected objective data for example, temperature, pulse, blood pressure, and respirations. The importance of patient/family teaching techniques introduced in new settings for example focused assessments at home directly relates to aid patients ability to recognize, identify, and relay presenting signs and symptoms to healthcare providers. Encouraging patients to utilize a focused assessment has potential to improve communication of health data from patient to healthcare provider and increase understanding of targeted health data by breaking down details to area of specific chief complaint.
