Effective October 11, 2022, Heartland Health Centers is now Tapestry 360 Health. Our mission is to improve the well-being of the communities we serve by providing accessible, high-quality healthcare. Our vision is to be a national leader in community-based health care by advancing innovative service models and patient-centered best practices. Tapestry 360 Health is a federally qualified health center with 18 locations serving Chicago’s north side and nearby suburbs. In 2020, we provided 27,000 patients with a medical home where we offer affordable and comprehensive primary care, oral health care, and mental health care services. As a medical home, Tapestry 360 Health ensures comprehensive, affordable, quality, safe, and coordinated health care that is culturally competent and orientated to addressing all the health care needs of each patient. We're recognized for delivering high-quality holistic health care, with awards from the U.S. We're accredited by the Joint Commission and certified as a Primary Care Medical Home. What is an FQHC? A Federally Qualified Health Center (FQHC) must serve an underserved area or population, offer a sliding fee scale, provide comprehensive services, have an ongoing quality assurance program, and have a governing board of directors. At just 15, Temika became a mother and faced challenges that could have broken many. Yet through strength, hope, and the compassionate care she found at Tapestry 360 Health, she began to rebuild her life. Her story, “Uncrumbled,” reminds us that healing starts with compassion — and that our mission extends far beyond medicine.

What an unforgettable evening! Our 10th Annual Gala, Blueprint for a Healthier Future, brought together 290 guests who believe in health equity, community care, and hope.

We were honored to recognize Patrick Thompson, of Manske Dieckmann Thompson Architecture Interiors, for his architectural collaborations that helped bring our vision for healing spaces to life, and Ollie Idowu, President and CEO of the Illinois Primary Health Care Association (IPHCA), for his leadership and advocacy on behalf of community health centers statewide.

🌏 Community in Action! Tapestry 360 Health was proud to participate in the Asian Health Coalition’s Health Fair at the Rizal Center over the weekend. This year’s theme, “Celebrating Our Cultures, Celebrating Us,” brought together dozens of community-based organizations sharing vital health and wellness resources.

Partners included HANA Center, Indo-American Center, Sanjeevani, Trellus , and many others. Walgreens provided vaccines—including COVID, flu, and pneumonia—helping our communities stay healthy and protected.

A big thank-you to event sponsors Pfizer and Penn State College of Medicine for supporting this impactful celebration of culture, care, and connection. 💙
