We are looking for a CMA/LPN to join our team as a Patient Care Manager/Chronic Care Management Nurse. In this role, you will help patients navigate the healthcare system by coordinating referrals, making follow-up calls, educating patients on medication adherence, and managing ER follow-up/hospital discharge calls. We are excited to announce that we have a new physician joining our group in late July. Dr. Gibson is board certified in family medicine and is qualified to care for a wide range of patients. As she joins our practice, she has a particular interest in seeing more pediatric patients so our group will begin offering more of those services starting in July.
