Health Care Access Now is a community-based care coordination nonprofit that assists and advocates for low and moderate-income consumers who do not have consistent and affordable access to health care.
Community based Care Coordination pairs individual, diverse needs with the complicated matrix of services in our community. Care Coordination services are provided by a specialized workforce – Community Health Workers. Through advocating, educating, connecting and supporting, CHW’s are partners with clients and the clinical and social services received in the community.
Health Care Access Now operates two Care Coordination programs:
Adult Health Care Coordination
Maternal & Child Health Care Coordination
Adult Health Care Coordination
Our Adult Health Care Coordination program sends a community health worker directly to an individual’s front door. We assist clients in taking away the barriers that would otherwise prevent them from getting routine care and managing their condition. We can attend doctor appointments, help with medication management and support clients in achieving their disease self-management care plan. Our goal is to be sure clients fully understand their needs and can maintain their health.
Maternal & Child Health Care Coordination
Expectant moms need more attention than they believe. Our Maternal & Child Health Care Coordination program assists moms during and after pregnancy to ensure they have a full-term baby and that mom stays healthy. We help primarily through education and addressing the needs of the pregnancy, after birth and through the first year of life.
Become a Community Health Worker
Health Care Access Now is a leader in developing Community Health Workers in Cincinnati, and beyond. We offer a 12-week Community Health Worker Certification Program which prepares individuals for certification through the Ohio Board of Nursing. All students are required to complete a community-based practicum that will enable one to be familiar with this new profession in the health care industry. New sessions begin through out the year. Scholarships are available.
