This Memorial Day, HCAN honors and remembers those who made the ultimate sacrifice in service to our country. As we reflect, we’re reminded of the strength of community—checking in on one another, offering support, and creating spaces where everyone feels seen and heard. Together, we can continue building a caring, connected community in their honor.

During Mental Health Awareness Month, we’re reminded that healing doesn’t happen in isolation. If someone you love is facing a mental health condition and substance use challenges, you are not alone—and neither are they. Support exists for the whole family.

Community Health Workers play a vital role in improving health outcomes—and the Pathways Community HUB Institute (PCHI) Model helps make that possible. As the regional director of the PCHI Model, HCAN Now supports care coordination that identifies and addresses participants’ needs. We’re proud to spotlight partner agency Atrium Medical Center , where CHWs are using this evidence-based model to create meaningful pathways to better health. 💙

Check out the Hamilton County 513Relief Bus Stops this month of April!

It may be April Fools’ Day, but this month’s 513Relief Bus stops are no joke.❕

We’re bringing resources directly to 12 communities across the region with 12 stops throughout April, connecting residents to support right in their neighborhoods.

Don’t forget our Youth Employment Kickoff happening Saturday, April 11 in the West End, creating opportunities for young people to access jobs, health services, and more.

Know someone in one of these 12 communities? Send them this calendar so they don’t miss a stop near them. 🚎
