Today we are excited to highlight our CHW from the East Central Ohio ESC!!!

"My name is Kaylyn Van Kirk, and I’m a Care Coordinator at the East Central Ohio Educational Service Center through the Bridges to Wellness program. I work closely with Cambridge City Schools to help bridge the gap between children, families, and the resources they need to thrive. Supporting kids and removing barriers to wellness is something I’m deeply passionate about. I’m also a proud mom of two wonderful kids and the Vice President of the local nonprofit Feed My People, where we serve free hot meals to our community Monday through Saturday from 10:00–11:30 a.m. Community care, advocacy, and showing up for others are at the heart of everything I do. "

Bridges to Wellness is so thankful for the United Way of Guernsey, Monroe & Noble Counties support to our Funder of Last Resort Grant. Due to them we are able to continue to serve individuals and link them to many social determinates of health. Please help us continue to support them by attending the event below!

I had the honor of presenting at the NewPointe Church Retiree Luncheon today, sharing the mission of Bridges to Wellness and the work we’re doing to support individuals and families in our community.
It was such a joy connecting with everyone, answering questions, and hearing the heart this group has for serving others. 💛

We just finished onboarding for the month of January!
Our first new CHW highlight is Mariela who is based out of the Cleveland Clinic Union Hospital OBGYN.
Welcome Mariela! We are so excited to have you onboard with us!! Read a little bit about her below.

Hi! My name is Mariela. I attended Buckeye Career Center Adult Education, where I earned my certification in Medical Assisting and gained hands on experience by shadowing in the healthcare field. I also worked at the YMCA as a Wellness Associate and Greeter—experiences I’m very grateful for, as they allowed me to learn a lot and connect with others. In my free time, I enjoy going to the gym and listening to music. I have also volunteered in outreach and translation services, which gave me the opportunity to support and engage with the community in meaningful ways. As a bilingual Spanish speaker, helping and supporting the community is my passion, and I am excited to continue making a positive impact as a Community Health Worker.

Bridges to Wellness is excited to announce that we have received grant funding for a "funder of last resort" in Guernsey, Monroe, and Noble counties for 2026!

But what does that mean though?

It means our CHW's in Guernsey/Monroe/Noble can enroll any clients regardless of insurance help link them to community resources.

We are thankful for our partner United Way of Guernsey, Monroe & Noble Counties for granting us this opportunity for the 2026 year!

Currently we have three CHW's in Guernsey County and one that serves Monroe and Noble jointly.

We want to extend our heartfelt gratitude to United Way of Muskingum, Perry & Morgan Counties for serving as the funder of last resort for our program.

Because of their commitment, Bridges to Wellness can continue serving participants who may not have insurance or have an insurance plan that we do not contract with.

Their partnership represents hope, stability, and a true dedication to community well-being. We are incredibly thankful for their belief in our mission and their ongoing investment in healthier, stronger communities.

We are so thankful for our Funder of Last Resort, United Way of Muskingum, Perry & Morgan Counties to help our Community Health Workers serve participants who may not have insurance or a plan that we are not contracted with!

We currently have 29 active participants being served within this grant. We are incredibly thankful for your continued generosity to our program!
