We are expanding our dementia services to include the GUIDE Model beginning July 1st. This will be an easy expansion for us as our local clinicians and care coordinators have already been providing these same dementia services for the past two years through our Dementia Support Complete and Healthy Brains programs. The GUIDE Model and our current programming focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. Our dementia services achieve these goals through a comprehensive package of care coordination and care management, and caregiver education and support. While our Dementia Support Complete/Healthy Brains programs are available to individuals with traditional Medicare, Medicare advantage plans, and commercial insurance, the GUIDE Model is solely for those with traditional Medicare. Some GUIDE Model participants may also qualify for a $2500 a year respite allowance. If you know of any of your clients that may benefit from these services, please have them call 513-563-0488.

We were so excited to co-host a Senior Resource Event with Care Patrol and Jewish Family Service again this year. On 4/16, at Jewish Family Service Adult Day, we had 24 representatives from a wide variety of community agencies that provide services to our local seniors. We had fantastic feedback from those who attended stating they appreciated the diversity of vendors and felt the information given was deeply helpful. Our community partners that participated were incredibly generous as well, donating personal care items to the Jewish Family Service Clifton Food Pantry. All table fees collected were used to make a generous donation to Giving Voice Foundation, a local non-profit, to continue their mission of inspiring cross-generational conversations and connection to improve the health of older adults through advocacy, education, engagement, and storytelling.

We were very excited to participate in Council on Aging's Forum on Aging 2025 on March 4th. Dr. Tim Sigward spoke to attendees about our selection as the only provider in SW Ohio to be a part of the CMS GUIDE program launching in July 2025. He shared how the GUIDE program focuses on comprehensive, coordinated dementia care for Medicare beneficiaries. It aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. Dr. Sigward also discussed our current programs aimed at providing assistance to individuals with dementia or mild cognitive impairment- Dementia Support Complete and Healthy Brains programs.
