Our experienced and caring staff is committed to meeting your family’s medical needs, including regular preventive care and management of acute and chronic medical needs. Horizon Family Medicine physicians undergo three years of formal training in an accredited family practice residency after graduation from medical school. Due to the upcoming winter weather and out of an abundance of caution, ALL Horizon Family Medicine locations will be closed on Monday, February 2nd.

The safety of our patients and staff is our top priority. With the potential for hazardous road conditions, we believe this closure is the best way to help keep everyone safe and off the roads.

ACC will resume as normal on Saturday 12/13 from 9am-1pm.
