INITIATIVE

The Coach Home Care lovingly provides personal care and homemaking services for clients in the client's home. Homebound clients often times cannot accomplish for themselves simple Activities of Daily Living (ADLs) due to physical, mental, or emotional circumstances. Personal Caregivers provide assistance with these activities in a friendly, self-motivated, timely manner.

The duties of the Personal Caregiver may include the following:

Observation and maintenance of the home environment that ensures the safety and security of the client.
Assistance with household chores including cooking and meal preparation, cleaning, and laundry.
Assistance in completing activities such as shopping, and appointments outside the home.
Companionship including, but not limited to, social interaction, conversation, emotional reassurance, encouragement of reading, writing and activities that stimulate the mind.
Assistance with ADLs, personal care, and any other assignments in the service plan.
Completion of appropriate service notes.
Specifically, these duties may include the following tasks:

Housekeeping
Laundry
Meal preparation
Shopping
Skin care
Ambulation assistance
Bathing assistance
Dressing assistance
Feeding/Eating assistance
Hair care
Toileting assistance
Transferring assistance
Medication reminding
Accompaniment
The Coach Home Care requires our Personal Caregivers to have some level of verifiable caregiving experience; in-home preferred, facility experience also acceptable. Applicants must have the following attributes:

Dependability
Promptness
Good attendance record
Trustworthy
Strong work ethic
Consistent availability
A valid driver's license, a dependable vehicle, and clear criminal and APS backgrounds are required.

Shifts from 2 to 5 hours/day available. Occasional 12-hour, overnight, and 24-hour shifts available.

Pay: $15.00 - 15.50 per hour

November is COPD Awareness Month

Chronic obstructive pulmonary disease, commonly known as COPD, is an umbrella term for both emphysema and chronic bronchitis.
COPD affects more than 24 million Americans, yet many don't even know they have it.
Emphysema is a chronic condition caused by damage to the lungs' air sacs (alveoli).
Chronic bronchitis is a long-term inflammation of the bronchi, the passage which brings air into the lungs.
Smoking is the primary cause of COPD, and quitting is essential to prevention and treatment.

Position summary:

Provides quality skilled nursing care to persons with disabilities and elders in their homes; works on team relationships with others and carries out assigned responsibilities. Performs client assessments based on agency and government standards of practice to ensure effective and appropriate homecare services. Directs, plans and initiates appropriate action independently and responsively in home care situations. Position will be paid per visit. Some case managing is required.

Minimum Qualifications:

Must have current license to practice as a Registered Nurse in the state of Colorado.
Current driver’s license, safe driving record and reliable transportation
Minimum of one year Medicare/Medicaid home healthcare experience.
Understanding and knowledge of developing care plans for client care.
Ability to establish and maintain constructive working relationships with individuals and their families.
Must be able to lift, push, pull up to 50 lbs. frequently.

We are looking for a committed and patient-focused Occupational Therapist to develop, recover or maintain the daily living and work skills of people with physical, mental or developmental conditions. You will offer effective and client-centered services that will enable people to confidently and independently carry out everyday tasks or activities. The successful candidate will be able to adapt to the environment to fit the person.

Responsibilities

Address client’s aspects of performance to support engagement in occupations that affect health, well-being and life quality
Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions
Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components
Assess client’s home and other environments and advice on alterations consistent with their needs and capabilities
Recommend adaptive equipment and train in its use
Guide and educate family members and caregivers
Observe, note, and report on progress of long and short term treatment goals
Collect data and document processes followed
Regularly follow-up client
Skills

Proven working experience as occupational therapist
Excellent knowledge of principles and practices of occupational therapy
Hands-on experience with computer programs and creative media
Familiarity with ADLs evaluation tools ( e.g. Katz ADL scale, Lawton IADL scale)
Ability to manage patients with different types of personalities
Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession
Professional behavior
Excellent interpersonal communication skills
Valid license to practice occupational therapy
BS degree or greater in occupational therapy
