In my work with Pikes Peak Discharge Clinic, we see that the most dangerous part of a patient's journey is the gap between the hospital bed and the home front. When vulnerable individuals fall through these "innumerable gaps," the entire system feels the strain.
PPDC is the local intervention. We don’t just monitor the recovery path; we manage it. We bridge the distance to ensure clinical data follows the patient and that their local care team is fully integrated from day one.
