HCRpath

HCRpath We help healthcare providers and patients on their path to better preventative health HCRpath is dedicated to improving healthcare for all seniors.

We believe this starts with making sure Medicare-covered seniors have access to preventative benefits that are included in their health plan. This access begins with the Annual Wellness Visit. We are committed to shedding light on the systemic problem of seniors—particularly those in minority communities—not getting access to the preventative care to which they are entitled. HCRpath simplifies the

Medicare Annual Wellness Visit and enables providers to deliver a better patient care experience helping integrate wellness visits into their service offerings easily and efficiently. HCRpath combines health and wellness screenings with a cross-reference of the patient’s Medicare-covered preventative appointment history.

"Increasing the frequency that a given senior meets with a primary care physician can improve that senior’s health, because the primary care physician comes to understand exactly how each chronic health condition is impacting the others."

If that isn't enough here are 4 other Reasons Why Onsite Primary Care is Important At Assisted Living Facilities

1. Primary care visits are crucial to managing chronic health conditions
2. Primary care visits are crucial to the success of ever-more-popular Medicare Advantage plans
3. Senior housing is becoming an ideal location for primary care delivery, improving health outcomes
4. Consumers are demanding that senior housing deliver and manage primary care

Meet our Founder and CEO, Sara Sable-Antry!

Sara is an entrepreneur who is tackling complex healthcare problems so patients can lead better, healthier lives.

HCRPath enables medicare patients to fill out their Initial Preventive Physical Exam (IPPE) or Annual Wellness Visit (AWV) survey digitally before their in-person visit.

Making it easier for you as the provider to collect their health data, listen to their story, and identify any health concerns.

Our surveys combine patients’ health stories to provide a customized plan based on Historical & Current health information, driving a care plan with proper expectations and measurable outcomes.

Spend less time stressing over paperwork and spend more time getting to know your patients. Our surveys create a unique experience for you and the people you care for. Adding touches of personalization and increasing resident (and their families) satisfaction.

"The standard monthly premium for Medicare outpatient, or “Part B,” coverage, will be $170.10 in 2022, up from $148.50 in 2021"

If you are going to pay more, you need to get more. Annual Wellness Visits have no copay through Medicare so no more money out of pocket but a great value to your health.

Assisted Living Administrators are lacking ways of aggregating resident data across their healthcare and ancillary providers, family members, and internal staff, leading to inefficient onboarding and care planning, resulting in lower resident satisfaction.

It's our goal to provide onboarding platforms (SaaS), like our surveys, that connect the resident, staff, Primary Care Physician (P*P), ancillary providers, and insurance companies providing an efficient, personal experience for each resident and making life easier for our overworked administrators.

Increases onboarding that provides a unique experience for the resident, their family, and referring physicians.

Let HCRpath make your experience better. For you and your patients.

Research has found that skipping sleep may lead to chronic disease.

Sleep is an essential function that allows your body and mind to recharge, leaving you refreshed and alert when you wake up.

Insufficient sleep has been linked to the development and management of a number of chronic diseases and conditions, including type 2 diabetes, cardiovascular disease, obesity, and depression.

Are your patients taking care of themselves and getting the sleep they need? If they aren't getting enough sleep is that what is leading to their anxiety and depression?

Our surveys can help you get to the root cause and catch the problem before it's too late.

"Regardless of a patient’s comfort level with technology, removing unnecessary steps during the identity verification process is the beginning of a positive digital experience. In essence, it makes patient onboarding simpler and faster for all."

We can't say it enough👏

Making things easier for your patients makes things easier for you. Simple onboarding = better experience. When your patients have a better experience it makes it easier for them and you to get to the things that actually matter, patient care and treating conditions.

Did you know that our founder had her own preventative health surgery?

In November of ‘21, she found out she was BRCA-1 positive.

She had a complete hysterectomy and an oophorectomy in February of 2022 and she will have a double mastectomy with reconstruction this summer.

We asked her what her thoughts were about these surgeries and why she decided to have them and this is what she had to say...

"I had the choice, quite frankly. How many women receive the devastating news that they have cancer and were not able to be proactive about it? Ovarian cancer in particular really scared me due to its high rate of death."

We also asked Sara about her experience working with healthcare providers and ease of care...

"I was very fortunate. A family member had genetic testing conducted to determine their best path forward for treatment options. It's amazing how much your DNA determines your ability to respond to treatments or metabolize certain medications or even the impact on dosing. My family members' results showed that they had the BRCA-1 gene so it was suggested that family members get tested as well. So on one hand, Genetics is very well organized and they are motivated to provide testing because there is a tremendous value for future research. They want you to get tested! But for other healthcare providers who do not have a clear reason to test, such as relative testing positive, it can be difficult to gain a referral for the full genetic testing. This means someone may have a long family history of cancer but without a genetic evaluation, there is little they can do but sit and wonder "when" or "if".

What really frustrates me is the sheer cost of the procedures. I have excellent health insurance, thanks to my husband, and I have the means to pay our deductibles and other costs. What is heartbreaking is when someone receives the news that they have BRCA-1 and they cannot afford the life-saving surgeries."

We are so excited to introduce you to another incredible woman in the medical community...Dr. Maria Lesia Strus, MD.

Not only is Dr. Maria Strus an incredible woman but she is also HCRpath's Medical Director!

Dr. Strus graduated from the Case Western Reserve University School of Medicine in 1999. She works in Cleveland, OH, and specializes in Emergency Medicine, Internal Medicine/Pediatrics, Internal Medicine, and Pediatrics.

We asked Dr. Strus why she chose to work with HCRpath as a part of her medical journey and here is what she had to say...

"Preventative care still and must be emphasized as the best mechanism to prevent serious (expensive) disease later, which in turn, will prolong a higher quality of life than that of someone battling serious illness.

When I first met Sara Sable-Antry, the founder of HCRpath, I saw a visionary with endless energy and ideas to address a problem she witnessed firsthand – inadequate preventative care.

Inadequate preventative care contributes to serious problems directly and indirectly related to the care that was missed. Her talented and dedicated team of professionals is working feverishly to make her vision come true – capture the market that is slipping through primary care’s grip and identify what they need and what is missing

HCRpath can provide a mechanism to “coach” people into preventative care or continued care of chronic conditions.

HCRpath offers an independent look into the healthcare “portfolio” of individuals to improve healthcare going forward

HCRpath is embarking on technological advancements to bring the personalized analysis of patient’s health

HCRpath so strongly believes in preventative health that many mechanisms are being developed to reach people of all levels, age groups, in different settings, and abilities

These are some of the reasons I was attracted to HCRpath. Each member of the team brings a gift to the "development" table and I learn from each of them as I hope they learn from me.

I was in particular drawn to Sara's leadership and visions for her future healthcare platform. Sara is a woman who has a keen eye for detail, understands the healthcare market as a whole and many aspects in detail, and is bringing those elements together to address the problem tugging at her heart - bringing primary care to the people who need it the most."

We are so honored to work alongside Dr. Strus and excited to continue to grow as a team.

As a woman, how many times have you heard..."Yeah, that just kind of happens at your age."

From the time women start their period they are told they just need to "deal with it," as women, we're taught the "suck it up" mentality. How long do women need have to have this mentality until we realize something is wrong?

Heavy bleeding, light bleeding, no bleeding, lack of sleep, hair loss, mood swings, trouble getting pregnant, headaches, body aches, nausea, acne, cramps, depression, anxiety, and the list goes on and on...

As a provider, you may see these individual symptoms and think it's an isolated situation but when we have the actual data you can find a much clear correlation between symptoms like the ones listed above, allowing you to provide preventative care instead of going into triage mode.

With HCRpath surveys you get a comprehensive history of every patient and you can see where normal stopped and abnormal patterns begin.

Our surveys encourage conversation which helps put the pieces together and start treating the problem.

Were taking the "that's just what happens at your age" and turning it into "tell me more..."

For women's health month we want to feature some of the amazing women in the medical community.

We are so excited to introduce you to Angela (Angie) Hoff, DNP, APRN, CNP, owner, and operator of At Your Door: Visiting Healthcare Services

She has been a Registered Nurse since 1993, with over 20 years in the home care industry. Angie has been a Nurse Practitioner since 2009 and has been providing primary care in a wide range of facility settings.

Sometimes it can be very difficult for senior living residents to get to their Primary Care Physician’s office. This is why Angie started At Your Door: Visiting Healthcare Services in 2015. With her passion for providing trusted, compassionate care, At Your Door has grown into one of the largest providers of “in residence” primary care services in the central Ohio area! Each of their providers is hand-selected to continue Angie’s passion and love for the senior population.

In Other News...

At Your Door: Visiting Healthcare Services continues to grow services into the Dayton and Cincinnati areas.

At Your Door is also in the start-up phase of their expansion to Florida! Their Business Development Director, Robin Ryan moved down in the late fall and has been working to spread the news about AYD! They hope to start seeing patients in the Tampa area this summer.

They continue to work on ways to improve their services. They have 2 wound-certified nurse practitioners, one part-time psychiatric mental health nurse practitioner (with hopes to add another soon!), and they are participating in two different alternate payment models as well as partnering with local hospitalist groups to round out our continuity of care.

Be sure to check out At Your Door: Visiting Healthcare Services and give Angie a follow on LinkedIn
