Operating as a Federally Qualified Health Center (FQHC), we offer services in primary care, pediatrics, behavioral health, substance use disorder, OBGYN, diagnostic laboratory, pharmacy, dietetics & nutrition, and interpretation

. With five locations in Ohio, HOFHC has a team of more than 90 providers and staff in three locations dedicated to providing quality health care to thousands of individuals and families.
