Orange Township firefighters took part in an OhioHealth cold-water rescue simulation, practicing how to reach and pull a struggling “victim” to safety.

Key EMS Priorities in Cold-Water Hypothermia

• Safe, controlled extrication: Prevent further heat loss and minimize rough handling to reduce the risk of precipitating arrhythmias in a cold myocardium.
• Immediate rewarming steps: Insulated blankets, v***r barriers, warmed oxygen if available, and heat packs to the axillae/groin/neck as appropriate.
• Airway & ventilation management: Hypothermic patients may have depressed reflexes; crews monitor for apnea and maintain gentle ventilation strategies.
• Continuous cardiac monitoring: Cold patients can present with bradycardia, Osborne waves, or dysrhythmias—slow is expected; handle the heart with respect.
• “Not dead until warm and dead”: Hypothermia can preserve neurologic function. Aggressive resuscitation is indicated until core temperature is adequate for accurate assessment. 86 degrees is a key temperature to remember!
• Warm transport destination: Coordination with the receiving emergency department for warmed IV fluids, active internal rewarming, and specialty resources.

In honor of today, we spent part of today's EMS Fellowship lecture series discussing changes within EMS and Disaster Management as a result of 9/11.
